What are the key principles and benefits of risk-based quality management (RBQM)?

RBQM should be carried out for all sections, departments process etc. This will help to reduce or mitigate the risk at all these areas.

RBQM is a a systematic, prioritized, risk-based approach to quality management in clinical trials. Focus is placed on proactive identification and mitigation of risks while concentrating on few critical factors which can influence the patient safety and data integrity. This approach involves establishing predefined thresholds, metrics, and tolerance limits to continually monitor risks and effectively communicate them to various stakeholders involved in a clinical trial.

Identifying potential risks and addressing them with mitigation strategies can minimize the company's direct and indirect losses. A good understanding of risk management can enhance the decision-making power which helps to move towards organizational goals. By focusing on resource management, one can maximize the overall impact of mitigation strategies to achieve organizational goals.

This risk based quality management approach is proactive like FMEA or Risk matrix which primarily focuses on risk identification on activities, processes, products and services including resources. Once risk identified then comes risk prioritization like HML (high medium or low) based on Severity, probability and impact. which in turn leads to formation of risk mitigation plan of accept, reduce, remove, partial, or share risks with other stakeholders using admin, engg. control or changes in processes and activities. review and revisit regularly risk re-evaluation for continual improvement. this will improve decision making, reduce cost, improve quality stakeholder trust, collaboration, reduce waste leads to growth of organization.

There are several benefits of RBQM. However, one benefit stands out: expanding the scope of the concept of risk in clinical trials. Traditionally, risk in clinical trials has been focused, and more often limited to harm and consequence to the study subject and data integrity. While this remains the core of the concept of risk in clinical trials. In many cases it has meant that other critical risk areas in clinical trials often have received less attention than they deserve. RBQM highlights the importance of identifying and focusing on critical process based on the risk profile. Consequently, new risk areas of focus are likely to emerge e.g. in DCT designs, supply chain disruption may be identified as one of the areas of focus.

True, based on our experience, implementing RBQM requires a significant shift in mindset. It's not just a procedural change, but a transformation in the way clinical trials are conducted. The main steps in Risk-Based Quality Management implementation are: 1. Identify Critical Processes and Data. 2. Risk Identification. 3. Risk Evaluation. 4. Risk Control. 5. Risk Communication. 6. Una recomendación es integrar enfoques basados en riesgos para: - Identificar datos y procesos críticos y sus riesgos. - Evaluar la probabilidad, detectabilidad e impacto de dichos riesgos. - Determinar si los riesgos son aceptables o si deben reducirse en función de límites preestablecidos. - Documentar y reportar riesgos - Revisar las medidas de control de riesgos periódicamente, para determinar la eficacia de las medidas de control.

En mi experiencia en proyectos EPCM puedo comentar que implementar RBQM requiere un liderazgo desde la dirección y un cambio en la cultura organizacional; esto para entender la RBQM como una herramienta y/o un proceso que permite anticipar y gestionar riesgos proactivamente. Esto implica formar un equipo, asignar recursos y tiempo. Es fundamental involucrar a todos los niveles de la organización, para establecer roles claros y fomentar una cultura de mejora continua y prevención.

For sponsors, risk assessment and RbQM aspects are central considerations when outsourcing services to a CRO. During the qualification audit, a thorough assessment of the CRO’s QMS should help the sponsor ensuring that RbQM is adequately considered and implemented. Description of requirements in SOPs is one thing, but the organisation should also be able to translate RbQM into processes and assessments. Remember: Sub-subcontracting is a risk! The CRO should therefore have established vendor oversight processes to make sure that quality standards are consistently upheld also in outsourced services. Subcontracting by your CRO should thus be part of the risk assessment during the planning phase of your study.

En mi experiencia puedo compartir lo siguiente La gestión de Calidad basada en Riesgos es importante si es bien realizada. Esto implica una estrecha combinación de experiencia en las actividades y habilidades en análisis de riesgos. La experiencia aporta conocimientos prácticos esenciales y determinantes, mientras que la habilidad en análisis asegura un enfoque riguroso y metodológico, por tanto es importante siempre formar un equipo multidisciplinario para identificar y/o entender riesgos específicos y al mismo tiempo poder estructurar y evaluar los riesgos de manera sistemática.
