In my experience, the scope is best defined visually. In all my proposals the first thing I do is map out the AS-IS process, identify the gaps, and then connect it to the TO-BE process with the solution. This helps the prospect and I get on the same page. If this is done visually as opposed to boring text heavy Executive Summaries, it works even better as humans are visual beings.

Will probably depend on the business, but in general it needs to start with background on the client and big picture goals, followed by some brief information about your firm and why you are even qualified to propose anything. Afterwards, you have to define the subject you are working on, major steps you will take to achieve the goal, needs in order to complete the work, timelines, people involved, deliverables and potentially options if there are different ways to approach the project. It should encompass the entire solution, but probably not so granular as to get into terminology the client may not understand or need to know.
