How do you handle situations where patients face barriers accessing essential medications or therapies?

An interdisciplinary care model is recommended for certain complex and chronic conditions. Nutrition support therapy (enteral and parenteral nutrition) is especially complex. Compared to other long-term chronic conditions, nutrition support has disproportionately high systemic barriers to access. This includes high cost of supplies, inadequate insurance coverage, limited reimbursement, and on-going national supply shortages. Other barriers include limited access to specialized care and specialists. Beyond that, there are disproportionate barriers to access for individuals from diverse socioeconomic and cultural backgrounds. When there are insurmountable barriers, complications can be moderate to severe, including morbidity and mortality.

We need to understand patients are very vulnerable to share about their challenges, so it's important to build trust during the moments with them. From my experience, a few things really help in building the trust. 1. Drop any assumptions and avoid thinking "I'm right, you're wrong." 2. Be curious and empower them to see what better health could look like for them 3. Be patient and celebrate small win with the patient. 4. Figure out how they learn best and give them the right resources. These allows me to understand children/family better, allow me to create the right tool to make medical topic fun and relevant for kids. It transforms into a authentic relationship and they are eager to learn more when they know you are on their side.

Barriers blocking access to proper healthcare could include: - Remoteness of patients’ residences from near HCPs - Absence of proper healthcare infrastructure - Failure to deliver genuine education on healthcare conditions, medications and prognosis - Low patient compliance, which is per se a multifactorial issue

Talk to the patient, find out what the limitations and barriers may be then find a workable solution to get them the help they need. If it is financial some pharmaceutical companies have programs to help those in need and this may be an option. Clear communication between provider and patient is essential in bridging the gaps in healthcare.

Collaboration: Work closely with social workers, case managers, and community organizations. Connect patients with resources (e.g., patient assistance programs, local clinics).

Exploring available options in such situations is imperative in providing care. Knowing what the issues are will help the provider to connect with the interdisciplinary team for resources that may be available. Social workers are great resources in knowing what is available and who to contact.

Advocate for patients by communicating with insurance companies, pharmacies, and providers. Also, help patients navigate prior authorizations and appeals.

Knowing what support is available in the community can make all the difference for the patient. They many not know what is out there. There are many charitable and support groups available that can assist the patient. This requires exploring the community resources and connecting patients to the right support systems.
