Where risk escalates is generally through unexpected changes to the agreed plan. The oilfield has a process for these eventualities called the "management of change". This can be done at junior levels, Roustabouts and Roughnecks all the way through to senior management levels, the level of approval that is needed to continue is derived from the risk the new or improvised operation arrives at, once the risk assessment is complete. A high percentage of problems that occur are to do with persons not using the company's systems, due to integrity under pressure issues or using them improperly, do to training experience issues. Simply put the safety processes must be more important than the work-scope to be executed or there is no process safety.
