The shift towards profit over progress in healthcare is a trend that concerns us all. I recently came across a poignant post from a healthcare professional, Jade Bartley, M.S. (Finni Health (YC W23)) in the behavioral therapy field that really struck a chord. Administrative tasks affect not just the staff and therapists but, most importantly, children like Max who need more time with their care provider. The scenario Jade describes, where supervisors are bogged down by administrative tasks and "productivity targets" overshadowing small but significant victories, is a stark reminder of what's at stake when care becomes just another line item on a balance sheet. However, there's a silver lining in offloading these time-consuming tasks to AI. Here are a few use cases of front-end task operations automated by Aarogram: 1. I got a call from a BCBA yesterday—tears in her voice. Her learner, a child we’ll call "Max," isn’t making progress lately. Sessions feel rushed, staff turnover is constant, and the team’s overwhelmed. The reason? Private equity (PE) took over the agency last quarter, and suddenly it’s about numbers on a spreadsheet, not outcomes in a therapy room. Before, supervisors and technicians spent meaningful time collaborating, refining intervention plans, and problem-solving. Now, the schedule is packed to capacity. Supervisors are scrambling—constantly weighed down by administrative demands they never had before. "Productivity targets" have replaced thoughtful parent training. Tiny behavioral gains that once lit up the entire team’s day are now footnotes in cost-benefit analyses. Max’s parents have noticed. They used to get tailored strategies and progress updates that made sense in their reality. Now it’s a five-minute handoff—sometimes with a new face they’ve never seen before. The trust they once felt is eroding. The heartbreaking truth? Quality care is often the first casualty when PE steps in. Instead of nurturing a child’s potential, the focus shifts to metrics: reduced costs, higher margins, rapid expansion. But we’re talking about human lives here—families who rely on consistent, compassionate support, not an assembly line of back-to-back cases. The best therapy happens when teams have the time and resources to understand each child’s story, build relationships with parents, and iterate on clinical plans. Leaders and BCBAs: If you find yourself under that PE spotlight, remember why you started this work in the first place. Ask: - Are we giving families our full attention or just running the clock? - Did we prioritize a child’s best interest—or the bottom line—when we last changed schedules or staff assignments? - How can we push back to ensure that "efficient" doesn’t mean "low-quality"? When private equity’s demands overshadow care, it’s not just Max who suffers. It’s everyone—therapists burned out, parents disheartened, and kids losing opportunities for growth. Let’s get back to what truly matters: delivering therapy that uplifts families and helps children thrive, not just meet a quarterly goal.

EHRs primarily came out as billing tools and were not made with workflows & efficiencies in mind.. explained Josh Lesko, MD at a US Navy Hospital in his interview with Todd Unger, for American Medical Association. Similar to his case, healthcare groups can pilot AI tools that are out there and test to see if the tool integrates well with case-specific workflows and check to see if these tools bring the threshold down to get administrative jobs done. Dr Lesko further urged physicians to be an active part of integrations and discussions on AI and pointed to resources offered by ACGME and AMA on the latest on AI in healthcare. He suggested that a simple Google search on 'AI in Healthcare' returns the latest trends and tools that can help us stay well informed. However, Dr. Brett Oliver, MD argues that health systems and medical groups first need to raise the level of their overall “AI literacy” before such an opening is created and that everyone should understand the basics of how AI works. Healthcare groups have resorted to creating an AI oversight committee with representatives from the organization’s clinical, operational, legal, finance, marketing and other divisions.
