About us
Adobe Population Health (formerly known as Adobe Care & Wellness) began as an idea for a healthcare solution dedicated to improving lives. Applying the human touch, Adobe develops real relationships forging a pathway forward improving both the human and health condition of each member. Adobe builds trusted relationships by meeting members where they feel safe and comfortable, in their own homes (or as we like to say, their adobe). Beyond identifying at-risk populations, Adobe proactively intervenes through a customized data-driven approach meeting compliance requirements by closing care gaps, addressing social determinants and supporting transitional care management. Adobe does more than check a box; the innovative Adobe Care Management Model delivers whole-person care that changes lives.

Ensure the health and productivity of your team by encouraging flu vaccinations. The flu vaccine reduces severe illness, hospitalizations, and long-term complications, helping to minimize workplace disruptions. Vaccination also protects vulnerable employees and contributes to community immunity. Many organizations offer flu shots onsite or provide access to free or low-cost vaccination clinics. Invest in the health of your employees today. Learn more: https://bit.ly/3Z5GCFA #EmployeeHealth #FluPrevention

Mental health and socioeconomic hardship often form a cycle that's hard to break, with individuals in lower income brackets disproportionately impacted by anxiety and depression but facing barriers to care. At Adobe Population Health, we're addressing environmental and healthcare barriers through our MASLOW survey, which identifies social determinants like food insecurity and housing instability. By providing resources such as transportation assistance, medication delivery, and affordable support services, we aim to foster mental and financial well-being in underserved communities.
