Communication and Trust in Health Reform: A Call for Collaborative Leadership When the message is clear but the messenger is ineffective, communication can break down. The Kenyan government is currently facing backlash over the discontinuation of the National Hospital Insurance Fund (NHIF) and the implementation of the Social Health Insurance Fund (SHIF). Much of this negative response is directed at President Ruto, who has earned the nickname "Kasongo" and is viewed as the face of these changes. Under the new constitutional framework, the health sector has been devolved, meaning that the national government should collaborate with county governments to implement any changes. The national government is expected to focus solely on the 47 counties. However, in what appears to be a power grab, the President has taken it upon himself to engage directly with citizens. Unfortunately, public trust in President Ruto has eroded, negatively impacting the success of any programs he leads. For any program or project to succeed, trust is essential. Both the Afya Yangu initiative and the Social Health Insurance Fund (SHIF) are struggling due to the lack of trust associated with him. While the technology and benefits of these programs are promising—requiring only an identification document and a phone number for registration—mistrust remains a significant barrier. The President should consider stepping back and allowing the Health Secretary to engage with the county health departments for a smoother implementation process. It’s a trade-off: if he withdraws from the healthcare ecosystem and lets the counties take charge, the programs may succeed, but he won’t gain political mileage. Conversely, if he seeks to leverage political advantage, the programs may drag on, ultimately costing him political capital. This situation represents a loss for the President, and he should strive not to make it a loss for the country. The Policy posture by @USCIS will likely change from family, victim, and Chance-based to Merit and Collaborator. Chain migration, citizenship by marriage, protection against foreign danger, and lottery green cards may have run their cause. The main reforms suggested for U.S. #Immigration, work authorization for professionals, and the #Asylum process include: U.S. Immigration Reforms: 1. Consolidation of Agencies: Merge U.S. Customs and Border Protection (CBP), Immigration and Customs Enforcement (ICE), U.S.Citizenship and Immigration Services (USCIS), the Department of Health and Human Services (HHS) Office of Refugee Resettlement (ORR), and the Department of Justice (DOJ) Executive Office for Immigration Review (EOIR) and Office of Immigration Litigation (OIL) into a standalone border and immigration agency. 2. Elimination of T and U Visas: Victimization should not be a basis for an immigration benefit. Instead, the S visa should be used for those actively and significantly cooperating with law enforcement. T and U visas are special types of non-immigrant visas in the United States aimed at protecting certain individuals who have experienced extreme hardship. 3. Clear Guidance on Detention and Bond: Issue clear guidance to prevent illegal aliens from bonding out and disappearing into the interior of the United States. 4. Prioritization of National Security in SEVP: End deference to educational institutions and remove security risks from the Student and Exchange Visitor Program (SEVP). Work Authorization for Professionals: 1. Merit-Based Immigration System: Create a system that rewards high-skilled aliens instead of the current system that favors extended family-based and luck-of-the-draw immigration. 2. H-1B Program Reform: Transform the H-1B program into an elite mechanism to bring in the "best and brightest" at the highest wages, ensuring that U.S. workers are not disadvantaged. 3. Mandatory E-Verify: Permanently authorize and make E-Verify mandatory to ensure that only eligible individuals are employed. 4. Fee Increases and Limitations on Fee Waivers: Increase fees for USCIS services to keep up with inflation and limit the availability of fee waivers. We anticipate the policy posture change to have a political high but have no effect on the net US immigration flows. The Southern border immigration is driven mainly by season agriculture labour which is not affected. The majority of African and Asian immigration into the US is merit-based on competencies and skills the economy needs. Prospective immigrants must start being picky on the recruitment agents since victim protections will depend on self regulation. We recommend Using certified ethical international recruitment agencies. Foreign Educated Health Professionals will continue being needed and CGFNS International, Inc. The proposed reforms for the U.S. Small Business Administration in President Elect's Project 2025 include: Leadership and Accountability: 1. Appoint a highly qualified SBA Administrator and leadership team. 2. Ensure regular reporting to Congress and timely responses to requests and recommendations. 3. End waste, fraud, and abuse in COVID-19 relief programs and enforce loan repayment or legal action for ineligible recipients. Program Focus and Efficiency: 1. Concentrate on congressionally authorized programs. 2. Eliminate direct lending by the U.S. Small Business Administration. 3. Support small-business lending and capital programs to enhance the small-business supply chain. 4. Implement nonduplicative programs on a first-come, first-served basis using the latest technology. Office of Advocacy: 1. Strengthen the Office of Advocacy with a renewed mandate and additional resources. 2. Amend the Regulatory Flexibility Act (RFA) to require agencies to consider the impact of regulations on small businesses. 3. Increase the Office of Advocacy's budget by at least 50 percent. COVID-19 Lending Program Accountability: 1. Finalize the cleanup of fraudulent COVID-19 loan and grant activity. 2. Consider private-sector support to close out these programs and enforce repayment for ineligible PPP loans. Disaster Loan Program: 1. Assess the extent to which another agency should offer disaster loans. 2. Explore private-sector channels for administering disaster loans. Religious Entities Eligibility: 1. Remove unconstitutional religious exclusions from SBA loan programs. 2. Finalize or update the proposed rule to ensure equal treatment for faith-based organizations. Small Business Innovation and Technology: 1. Continue and expand the SBIR and STTR programs. 2. Ensure stricter rules for SBIR funds to be used for capital investments in the U.S. Domestic Manufacturing Support: 1. Reform the Section 7(a) loan program to support manufacturing facility construction and equipment upgrading. 2. Refocus the SBIC program to support small businesses, especially in capital-intensive sectors. Small-Business Size Standard Modernization: 1. Encourage the creation of a "medium-sized business" classification for access to capital programs. Legislative Support: 1. Support SBA reform legislation that aligns with these proposed changes. 2. Advocate for legislative initiatives that focus on core statutory activities like capital access, federal contracting opportunities, and regulatory advocacy. These reforms aim to make the SBA more effective, accountable, and focused on supporting US small businesses in the modern economy.
