𝐂𝐌𝐒 𝐒𝐭𝐞𝐩𝐬 𝐔𝐩 𝐟𝐨𝐫 𝐑𝐞𝐭𝐢𝐧𝐚 𝐏𝐚𝐭𝐢𝐞𝐧𝐭𝐬, 𝐀𝐬𝐤𝐬 𝐈𝐧𝐬𝐮𝐫𝐞𝐫𝐬 𝐭𝐨 𝐏𝐚𝐮𝐬𝐞 𝐒𝐭𝐞𝐩 𝐓𝐡𝐞𝐫𝐚𝐩𝐲 Today, the Centers for Medicare & Medicaid Services (CMS) issued a memo encouraging Medicare Advantage plans to temporarily suspend Avastin-first step therapy policies. This important decision is a direct outcome of ASRS and AAO advocacy efforts, including a joint letter and subsequent meeting with CMS to address the challenges retina practices face following Pine Pharmaceuticals’ exit from the market. The memo also reaffirms the requirement for MA plans to respond to expedited step therapy exception requests within 24 hours, ensuring timely care for patients. Thank you, CMS, for your responsiveness to this critical issue, and thanks in advance to Medicare Advantage plans for prioritizing retina patients and helping preserve their vision. Paul Hahn, immediate past chair of the ASRS Health Economics Committee, recently presented a newly published study from the committee that underscores the critical need for physician payment reform. The study found that since Medicare physician payment is not tied to inflation, reimbursements for commonly billed retina CPT codes are about 25% lower in 2024 than in 2015 when adjusting for inflation. These findings match trends seen across medicine. The data doesn’t lie. ASRS joins with the entire medical community in advocating for immediate action on physician payment reform. #StoptheCuts #PhysicianPaymentReform

Medicare Open Enrollment is underway. Retina patients should consider several factors before signing up or switching plans to ensure continued access to their retina specialist and needed medications. Consumers should also ask if an insurance plan requires preauthorization for any tests or procedures or if it requires step therapy for any medications. “Fail first” step therapy is a policy Medicare-eligible retina patients should be aware of as it can affect a patient’s ability to receive treatment recommended by their retina specialist.
