I present today my George Orwellian theory on rotator cuff pathology. Just like in Animal Farm, "All rotator cuff pathology is not created equal." Why is rotator cuff surgery so interesting? Because on any given day no two rotator cuff pathologies and subsequent surgical solutions are the same. If you noticed I did not use the term rotator cuff tear. Neither of the two cases I am sharing had a traumatic injury. The majority of rotator cuff pathology is not a traumatic "tear" but rather a biologic failure of the local environment. Case 1: 75 year old woman with greater than one year history of pain with failed conservative management. As you can see on her MRI she has essentially a full thickness failure of her supraspinatus confirmed on arthroscopy with a classic external impingement with high grade abrasion and thickening of her CA ligament, with exceptionally thin tendon tissue and a 1 cm hole in her cuff over the footprint. I elected to perform a minimalist repair with two Dynacord strands and a Healix advance self punching lateral row anchor. As I present on rotator cuff augmentation, I usually get the hand raise from the audience to say " I don't need to augment this small "tear" because I can fix it well." My reply is why put the cuff back into the same place with the same failed biology and expect a different result. I always augment this type of cuff repair. Here I used the Anika Integrity HA implant for a broad low profile coverage to add biology and increase the chances of healing. Case 2: 50 year old woman one year history of pain and failed conservative management. MRI demonstrates a 50% partial articular tear of the supraspinatus midsubstance. This is not a PASTA tear. Her enthesis is intact. This was confirmed arthroscopically. Are you taking this cuff down?? I am not. I elected to augment her tendon to increase thickness with the low profile Anika Integrity HA implant. William (Bill) Walsh has clearly shown the histologic data confirming full thickness tendon healing with this implant. Sorry not sorry Rotator Cuff Gurus from around the world, historically we have not been good at rotator cuff surgery. Rotator cuff failure is not simply a mechanical problem, it is also a biologic problem. So I ask, what are you doing to alter the biology.
