HIP PAIN S/P PROXIMAL FEMORAL OSTEOTOMY IN 27F This case will be debated at the Anterior Hip Foundation (AHF) 2025 “Appetite for Disruption” in Nashville, TN, June 6-7, 2025. PROCEDURE: TOTAL HIP ARTHROPLASTY WITH ANTERIOR APPROACH Here is the link to the clinical presentation and the pre-op imaging: https://lnkd.in/gEFrdGMp HPI: This is a 27-year-old female with right hip pain. She endorses a limited range of motion and states her right leg is longer from prior surgery as a child. She denies infectious symptoms, and her prior infectious workup has been negative. PMH: Past medical history is unremarkable. Past surgical history is significant for right proximal femoral osteotomy as well as right distal femoral lengthening osteotomy. PE: On physical exam, inspection shows a well-healed proximal lateral incision and a well-healed distal femoral lateral incision. Her range of motion of the hip shows flexion of 85°, extension of 10°, external rotation of 20°, and internal rotation of 10°. Her right lower extremity is LONG from previous distal femoral osteotomy lengthening. Her neurovascular exam is normal. How would you manage this patient? We’ve returned to Joel Matta’s journey to the Anterior Approach in the latest blog post. He shares how he refined the approach by adapting tools to be more effective (with the help of his trusty race car shop, naturally). He also covers his emphasis on transparency and the opposition this created: His background in acetabular and pelvic fracture surgery taught him to share all complications, including those that occurred during the development phase. However, this transparency promoted a backlash against the anterior approach. Critics eagerly highlighted, and often exaggerated, complications. But this wasn’t his first time experiencing opposition. He cheerfully continued to champion the approach.
