In this role, you will evaluate and treat home health patients under a physician-approved plan of care, collaborating with an interdisciplinary team to help patients achieve their goals. ➡️ Qualified candidates must hold a current RN license/registration in the state of hire, with one year of nursing experience preferred. While prior hospice experience is also preferred, it is not required.
