California is making impressive progress implementing the DxF! So far: 📝 98% of required health plans have signed 🏥83% of required General Acute Care Hospitals have signed 🧑⚕️83% of required Skilled Nursing Facilities have signed

The Data Exchange Framework is one step closer to a Healthy California For All by: ✅Seamlessly connecting health and social services providers for coordinated care  ✅Improving outcomes for patients across CA  ✅Advancing equity by addressing the needs of underserved communities

2024 was a big year for the DxF! Today, there are over 4,200 DxF participants across all 58 counties in the state, bringing timely, whole-person care to Californians everywhere. 🏥

As the year comes to a close, we’re reflecting on all the progress made with the Data Exchange Framework. This year, we’ve 🤝Brought together health providers for coordinated care  🏥Enabled timely access to patient records to give providers a clearer picture of a patient’s health history  💡Increased patient access to their own health data, empowering more informed health decisions
