Cancer Support Community San Francisco Bay Area and BASS Cancer Center collaboration enabled psychosocial distress screening at scale using National Comprehensive Cancer Network® (NCCN®) Distress Tool including identification and resolution of patients’ unmet health-related social needs. Nurse and Patient Navigation standards include increasing cancer patients’ access to psychosocial support. - Both negative and positive distress screen patients had self-reported unmet needs. Of the 70% who screened negative for distress, over one-half had identified unmet needs that may have been left unresolved by many hospitals’ distress screen processes. - Collaboration with CSC provided Cancer Center with critical resources to address 71% of the Problem List concerns. - CSC expanded the number of individuals consenting to receive ongoing communication on programs for the 63% of screened patients who had identified unmet needs. - 24% of screened patients participated in CSC programs in the 6-month period. There was a negligible number of participants from the Cancer Center prior to the study.
