  • Earlier this week, America's Physician Groups (APG) Fall Conference attendees heard from Dr. William Torkildsen, MD, chairman of Valley Organized Physicians (VOP), and Susan Harriger, CareAllies clinical program manager of social drivers of health, transitions care, and diabetes education about their innovative program addressing social determinants of health. Together, they showcased how leveraging clinical and claims data has led to significant improvements in medication adherence, decreased hospitalizations, and reduced medical costs.   • As Health Literacy Month concludes, we want to highlight a practical solution that’s making a real difference for patients. Dr. Armando Hinojosa of SeniorMed - part of Valley Organized Physicians - saw how many of his patients struggled to understand their insurance benefits. To address this, he created a one-page guide in English and Spanish and brought brokers on-site to answer questions in real time. This practical approach is a model that can be replicated by other physicians working with underserved populations.
