Congratulations to the first graduating class of Santa Barbara County’s Pediatric Concurrent Care Program, Partners for Kids! CenCal Health recently had the honor of hosting a graduation ceremony for 50 dedicated healthcare professionals from nine local organizations who completed a yearlong training program to support seriously ill children and their families. The program offers comprehensive and compassionate care for children from birth to age 21. “We are excited to introduce Partners for Kids to families in our community,” said Kieran Shah, CHPCA, President and CEO of VNA Health, and Chair of CenCal Health’s Board of Directors. “This program represents a holistic approach to pediatric care that prioritizes not only the treatment of the disease but also the well-being of the child and family. One of our incredible CalAIM partners, Doctors Without Walls - Santa Barbara Street Medicine (DWW), is changing the lives of individuals who are chronically homeless in Santa Barbara County. In partnership with CenCal Health, DWW is projected to double the number of CenCal Health members they are providing Enhanced Care Management services for this year. Through this CalAIM benefit, DWW’s clients receive additional support and coordination from a dedicated care team to address their complex medical and social needs. What an inspiring day for the Coalition for Upward Mobility! 🌟 CenCal Health was honored to support our coalition partners in planning the inaugural Upward Mobility Summit, which addressed poverty in Santa Barbara County.
