A career-defining challenge for retinal surgeon, Murtaza Adam, M.D. In this high-stakes case, a 30-year-old monocular male with a complex ocular history presented a unique and incredibly challenging surgical scenario. The patient, with history of Stickler’s syndrome and pediatric retinal detachment repairs in both eyes, leaving him with a prosthetic right eye and a severely compromised left eye. To make matters even more complicated, his left eye — the only remaining functional eye — was aphakic and aniridic, with a dislocated Morcher Aniridia Ring-IOL-capsular complex. This case required not only exceptional technical surgical skill but also immense psychological resilience. For vitreoretinal surgeons like Dr. Adam, the pressure to preserve sight in such a delicate and high-risk situation is palpable. The emotional weight of knowing that failure could result in permanent blindness for the patient underscores the importance of precision and perseverance in our work. Image of the month: Coat's Disease. Captured the talented retinal photographer, Frank Flanagin. Coats' disease is a rare, unilateral retinal disorder, typically diagnosed in males between ages 8-16, characterized by abnormal retinal blood vessels which can lead to exudative retinopathy and vision loss. Symptoms include decreased vision, strabismus, and leukocoria, with more severe cases showing exudative retinal detachment. The condition presents with dilated and tortuous retinal blood vessels, exudates, and hemorrhage in the peripheral retina. Staging based on funduscopic features helps monitor progression and guide treatment. Treatment options can range from observation in mild cases to laser treatment, cryotherapy, and anti-VEGF injections in more severe stages. In the most advanced cases, vitreoretinal surgery or enucleation may be indicated if the patient has a painful, blind eye. If left untreated, Coats' disease can lead to retinal detachments, glaucoma, and potential loss of the eye. Early detection and intervention, including thermal laser and cryotherapy, are key to improving patient outcomes. #CoatsDisesase #ophthalmology #retina #ophthalmologist

Today, Fran focuses on scheduling surgeries and specialized testing for our patients, but what has remained constant is her passion for working with people.
