Our Community HealthChoices (CHC) team was out representing CBH at the Together We Rise conference in King of Prussia last month. CBH’s CHC Team was created to improve the coordination of behavioral health care for seniors (65+) and other members with permanent physical disabilities who are dual-eligibles— covered by Medicare and Medicaid—under Pennsylvania’s CHC program. Our CHC Team is improving the quality of CHC members’ lives by ensuring access to behavioral health services and giving them opportunities to be involved in their communities and families. CHC provides professional development and conducts community outreach to nursing facilities, home healthcare, the Philadelphia Corporation for Aging, and senior living agencies to support behavioral health resources. Parents accessing behavioral health care may face emotional, physical, and financial challenges that can create barriers or gaps in obtaining treatment services. To help bridge these lapses in care, CBH established its High-Fidelity Wraparound (HFW) Program team in 2017 to focus on children and young adults with a behavioral health diagnosis, intensive behavioral health services, and extensive system involvement. The HFW team works with families to ensure that children can safely remain in the community, with their families, whenever possible. The team helps families identify preventive measures to reduce the likelihood of future crises and helps youth and families connect to their communities through school, work, and other activities.
