Karen Serrago M.S., from our corporate affiliate Community Technology Cooperative, had a great time meeting Massachusetts Governor Maura Healey at the Statehouse, during the recent signing ceremony for a bill that caps some prescription drug costs at $25, and also closed some of the loopholes that for-profit healthcare organizations were using to the detriment of their patients. About us
Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

To all of our providers, We are currently looking to add members to our very active Patient & Family Advisory Council (PFAC), which is a partnership between us and our patient and family member advisors, dedicated to advancing comprehensive and compassionate patient and family-centered health care for the myriad diverse populations all of us serve. The PFAC meets quarterly, with the express goal of improving the patient and family care experience by identifying opportunities, gathering, and providing feedback and perspectives on the plans, activities, consumer-oriented materials, and programs of C3 related to patient and family-centered health care. What are the responsibilities of PFAC members? - Partner and collaborate with fellow council members, as well as C3 staff, patients, and families, to ensure a patient and family-centered care experience. - Facilitate changes in care where, and when appropriate, to support the achievement and continued improvement of patient and family-centered care. - To always provide constructive feedback, from a PATIENT and FAMILY perspective. - Listen to the diverse opinions of others. - Adhere to the Confidentiality Agreement. - Review the materials received prior to each meeting, so you will come prepared to ask us questions, give us YOUR ideas, and provide us with YOUR input. - Make decisions based on consensus. When consensus is not reached, decisions will be made by a majority vote of ALL members. - A willingness to serve on the board for a minimum of two years. - Attend a minimum of three meetings a year. (There are only 4 in total.) - Have a willingness to serve on sub-committee activities when necessary. For every meeting attended, all PFAC members will receive a $75 gift card.
