Great Health Is Our Primary Purpose
About us
Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

How can you, as a leader at a Federally Qualified Health Center, benefit from partnering with our teammates at Community Technology Cooperative (CTC) if you need to enhance your Electronic Health Record capabilities? CTC: 1. Has a resolute long-term support team. Which means you do not have to staff EHR analysts, clinical builders, or report writers. 2. Manages all new-hire onboarding. Not you. 3. Fulfills all in-house training and support. Not you. 4. Provides 24-hour Epic help desk support. 5. Includes all third-party applications in your monthly operating costs. This allows you to take advantage of CTC’s economy of scale and pricing. 6. Our instance of the Epic platform is designed specifically to benefit FQHCs. And we are a company that is founded, and entirely managed, by our FQHC partners. All workflows are made for FQHCs.
