If you need care while on the road, Care Everywhere—our interoperability software—helps your doctors understand your medical history to provide care that’s most appropriate for you. No need for you to remember every medication, look up the names of previous doctors, or fax cumbersome files.     When patients can’t qualify for Social Security disability insurance because they can’t share their medical records, everyone loses.   ▪️ Patients accrue bills while they wait for disability determinations.   ▪️ Healthcare organizations, trying to help their patients, decrease their financial sustainability by providing uncompensated care while disability decisions are delayed.   ▪️ The Social Security Administration’s claims backlog grows from the inefficiency of manual exchange, surpassing 1 million pending review as of 2022.   In a new op-ed, Epic and the National Organization of Social Security Claimants’ Representatives (NOSSCR) explain the solution—including how health systems already use Epic to expedite determinations for patients in need.   Read more: https://lnkd.in/gEAMkvsn   #socialsecurity #interoperability #TEFCA

    Sepsis is a leading cause of death in US hospitals. Treating it effectively—saving lives—demands prompt intervention. Since establishing a dedicated sepsis team and creating a sepsis prevention and survivorship program, Lee Health has reduced the average length of stay for patients with sepsis by two days and has reduced mortality by 18%—or 110 lives saved in one year. The program follows guidelines from the CDC’s Hospital Sepsis Program Core Elements, uses Epic’s Early Detection of Sepsis cognitive computing model to help identify at-risk patients, and leverages remote patient monitoring to help patients care for themselves after leaving the hospital.     A pregnant person’s risk for complications doesn’t end with delivery—1 in 7 people develop a high blood pressure disorder during or after pregnancy, putting them at risk for stroke or organ damage.   M Health Fairview keeps an eye on postpartum patients with elevated blood pressure after they’ve left the hospital with a six-week home-monitoring program through MyChart. Patients record their blood pressure twice a day, and a team of nurses monitors that data and intervenes when necessary.   One year in, the program has served 367 patients and has led to a 69% reduction in postpartum readmissions due to hypertension.     The first three years of development have a lifelong effect on a child’s physical, social, and emotional well-being. That’s why it’s important to monitor children for developmental delays and intervene early.   NYC Health + Hospitals, in partnership with the NYC Health Department’s Bureau of Early Intervention, has worked to increase detection of delays for all children by integrating routine developmental screenings into pediatric visits in Epic. Through this and a simplified referral process for clinicians, they’ve increased early intervention referrals by 30%.     Thousands of Veterans have learned they’re eligible for healthcare, food, and housing benefits through enhanced interoperability between the U.S. Department of Veterans Affairs and our customer community. Here, Tufts Medicine and Sanford Health join Anthony Corso and Nick Frenzer—both Veterans and Epic staff—to share the why and how. “I’ve seen firsthand how critical access to care can be, having lost friends who struggled to find support after returning home from deployment,” said Anthony Corso, a combat Veteran and leader of Epic’s interoperability efforts with the VA. “Veterans have earned excellent hands-on care from their communities, and this is one way to help ensure that they receive it.” Here’s to helping those who have given so much—today and every day.     There’s no place like home—and the research agrees. Patients in hospital at home programs experience fewer complications and better recoveries than they would during long hospital stays. Cambridge University Hospitals NHS Foundation Trust, Manchester University NHS Foundation Trust, and Royal Devon University Healthcare NHS Foundation Trust established virtual wards to allow patients to receive hospital-level care from the comfort of home. Nurses monitor patients’ vitals using in-home devices, and they can offer in-person check-ins when patients need a hand with treatment. Across the three organizations, 94%-97% of patients reported high satisfaction with the programs. By the way—hospital at home programs are good for hospital capacity, too. Cambridge University Hospitals treated 1,800 patients at home in just one year, freeing up more than 4,000 hospital bed hours for higher acuity patients. Congrats, everyone!     Surgeons at Amsterdam UMC recently became the first in the world to test predictive modeling’s ability to help prevent complications during surgery. The model, which has been trained on data from 5,000 operations and has tested with 97% accuracy so far, is designed to predict which colon cancer patients are most at risk of complications—specifically “seam leaks,” which are common during bowel surgery. The model adjusts its predictions in real time based on data in Epic, including changes in the patient’s blood pressure or blood loss during surgery, and gives surgeons information to help them decide which surgical options might have a better outcomes. While the model is still in a pilot phase with more testing and validation needed, the team is enthusiastic about its potential to support surgical decision-making and believes it will be another useful tool in a surgeon’s tool belt.
