But here's the trap: When you say yes to everyone, you're actually saying no to something bigger. No to excellence. No to focus. No to becoming the best at what you do best. "But they need help," you say. "But I can do the procedure," you argue. "But my schedule has an opening," you rationalize. Here's what specialists forget: Being special means being specific. The best spine surgeon in the world doesn't do knee replacements. Not because she can't. Because she won't. Your specialty isn't a menu of services. It's a promise. A stance. A commitment to being the best at something specific. When you treat your specialty like a buffet, you become fast food. When you focus like a laser, you become a lighthouse. The paradox? The more patients you turn away, the more will seek you out. The more you say no, the more valuable your yes becomes. The more specific your promise, the wider your impact. Because true specialists aren't known for what they do. They're known for what they won't do. When was the last time you said no to good to make room for great? Robert Masson MD, FACS FABNS is one of my favorite examples of specialty spine health. Particularly, the genealogy of surgical evolution. Every generation of surgeon comes from a pedigree that was the trunk of a big tree at one point.
