  • A recent study in JAMA American highlights that even modest weight loss can result in significant cost savings. Researchers at Emory University analyzed data from over 17,000 individuals, some of whom had private insurance others had Medicare. The findings are eye-opening: Individuals with private insurance reduced their annual healthcare spending by 8% with 5% weight loss, while those who lost 25% saw a 24% reduction. Similarly, those on Medicare saw cost savings of 7% with 5% weight loss and 31% with 25% weight loss. Research findings like this adds to the growing body of clinical evidence showing that treating obesity not only improves comorbidities but also leads to meaningful patient outcomes and substantial healthcare savings. She outlines additional reasons for concern that were detailed in the paper recently published by American Diabetes Association. After reading, she concludes, "Compounded anti-obesity medications are just not a safe option for patients."     The American Diabetes Association published a paper this week on compounded GLP-1 and GLP-1/GIP receptor agonists. I have written about this before, but I learned so many crucial tidbits that I need to share. They come out very explicitly saying "Compounded GLP-1 RA and Dual GIP/GLP-1 RA Therapies Are Not Identical to the FDAApproved Products". ✅ It turns out that to compound a copy of an FDA-approved product one needs to follow a U.S. Pharmacopoeia (USP) monograph to ensure product quality, including its strength, purity, and performance. Because semaglutide or tirzepatide have not been around long enough, USP monographs are not even available! The compounders don't have the standard protocols. Yikes. ✅ When compounded products incorporate additional ingredients (vitamin B12 or B6), that inherently changes the composition. Compounded products are not subject to bioequivalence testing like FDA-approved meds, so we don't know the impact of added ingredients on efficacy, and safety. Not been tested! And the salt forms of semaglutide are definitely not chemically identical to the non-salt form, which could also result in clinically meaningful differences in the effectiveness and safety. ✅ There is an entire section on the reports of dosing errors and adverse events that have already been reported. All of the above have nothing to do with 503B vs 503A pharmacies. Not that I needed more convincing, but compounded anti-obesity medications are just not a safe option for patients. We need to continue to highlight the science, because the consumer market is exploiting the commercial opportunities at the expense of people with a chronic metabolic disease, and when you get down into the details, this is even more frightening!

  • New research published in the NEJM adds to the growing evidence that tirzepatide can significantly improve cardiometabolic health. The latest findings show that tirzepatide reduced the risk of death and worsening heart failure compared to placebo, in a population of individuals living with obesity and advanced heart failure. These results reinforce that anti-obesity medications like tirzepatide can improve health outcomes, even in very sick individuals. Addressing obesity, a root cause of many cardiometabolic conditions can also help control the long-term costs of other chronic diseases.     We are on a roll this week with new science on the effects of novel anti-obesity medications and the impact on cardiometabolic disease! Another randomized controlled trial published in the NEJM. Hundreds of patients with obesity and advanced heart failure were studies - this is a very sick population of patients. Treatment with tirzepatide (the dual GLP-1 and GIP receptor agonist) compared to placebo led to a lower risk of the composite end point - death from cardiovascular causes, and worsening heart failure. The clinical health status and function improved in those who got drug as well. This adds to the list of ways in which this class of drugs helps even the sickest people in the population get healthier. And it ties in to making the economic case for treating obesity, as the list is growing of all the health costs that are avoided downstream when we prevent and treat cardiometabolic disease progression.
