Care Innovation Grantee! Tanzania faces significant health challenges, with malaria being a leading cause of illness and death, particularly in rural areas. Despite efforts like insecticide-treated nets and antimalarial treatments, the disease remains widespread. HIV/AIDS and tuberculosis also affect many, with high HIV rates exacerbating TB transmission. Non-communicable diseases like diabetes and hypertension are rising due to urbanization and changing lifestyles. Limited healthcare access, especially in rural areas, and a shortage of trained professionals affect progress. There is a high mortality rate that remains a critical concern, despite ongoing improvements in healthcare services. Wekeza is a non-profit organization located in a village within the region of Mwanza, Tanzania. Mwanza is a port city that lies on the southern part of Lake Victoria, making it a malaria-risk location due to the humidity attracting mosquitos. Wekeza contains a hospital facility with paid and unpaid doctors who offer treatments and necessary procedures for low-income communities. These patients are typically from the llemela, Nyamangana, and Mwanza regions of Tanzania, as well as immigrants that pass through the region.

Honduras continues to face a humanitarian crisis, with 290,902 refugees and migrants entering, registering, and transiting through the country between January and August 2024—a 16% increase from the previous year. Health challenges for refugee and migrant populations are significant, driven by violence, poverty, and climate change. Many embark on perilous journeys in search of safety, facing health risks along the way, with access to healthcare remaining limited. Legión de Honduras is a nonprofit organization based in Tegucigalpa, the capital of Honduras. Honduras has seen a rise in mixed movements, with many individuals transiting through the area due to forced displacement. They provide free healthcare services to their communities and have their own equipped clinic to conduct medical brigades bringing healthcare directly to patients #digitalhealth #ehr #honduras #hikmahealth

Care Innovation Grantee! Ghana is experiencing significant challenges in its healthcare system, with over 30% of the population unable to access essential health services. The maternal mortality rate stands at approximately 319 deaths per 100,000 live births, and rising infectious diseases like malaria and cholera further strain vulnerable communities. Economic instability and inadequate sanitation contribute to these challenges, with nearly 24% of the population living below the poverty line. Additionally, more than 14,000 refugees struggle to obtain essential healthcare, ampl ifying their vulnerability. Health and Community Empowerment Project (HACEP-Ghana) is dedicated to meeting the urgent healthcare in the Upper East Region, particularly in areas affected by prolonged conflict. The Maternal and Child Health Mobile Clinic, established by HACEP-Ghana during the COVID-19 pandemic, addresses the healthcare needs of marginalized populations in conflict-affected and remote areas. The clinic provides essential services such as prenatal and postnatal care, family planning, vaccinations, and health education to more than 10,000 women and children annually. #ghana #ehr #digitalhealth
