☄️ Draining an Aortic Abscess Post-EVAR - yes or no? Proud to share a challenging and highly unusual case! Our patient, previously treated with EVAR for an aortic aneurysm a year ago, returned with severe back pain. A CT scan revealed a collection (yellow arrows) with air-fluid level (orange arrow) encasing the aortic prosthesis—an abscess ⚠️ ⚠️ Working alongside my colleagues in cardiovascular surgery, we determined the best course was a CT-guided percutaneous drainage. Here’s a look at the process: the first image shows the diagnostic CT scan and the final result with successful evacuation of the collection (had purulent aspect in the tube); additional images illustrate each step of the tube placement procedure. Retroperitoneal Adenopathie – Another Challenge Successfully Navigated Patient with history of breast cancer is found at follow up with a retroperitoneal adenopathy. We decided on a combined approach for biopsy- US and CT (for control during the procedure) Lesion was in close contact with the inferior vena cava. We used a transhepatic trajectory. (image 1&2) Thanks to careful planning and teamwork, the procedure was a success, and the patient was discharged just a few hours later with no complications 👏. 📌 Key takeaway? 🔍 Rare and Challenging Percutaneous Biopsy Case 🔍 We recently encountered a rare case involving a young patient with a tissue infiltration that encompassed the kidneys and major vessels, including the aorta and IVC. The differential diagnosis included Erdheim-Chester disease, lymphoma, and retroperitoneal fibrosis. We decided to perform a percutaneous core needle biopsy with a 18Ga needle targeting the tissue surrounding the kidneys. In the video you can see both the anesthesia and the biopsy. While we eagerly await the histopathology results, we’re excited to share this rare case with our network. Special thanks go to our colleagues in the cardiology department for referring the patient to us. 🔹IR in pain management -celiac plexus block for pancreatic cancer🔹 Sharing a case where a patient with a cephalic pancreatic tumor (first image) was experiencing significant pain. To help with the symptoms, we performed a celiac plexus block. We approached this with the patient in ventral decubitus, using both US and CT guidance to position a 22 Ga needle (second image). As neurolytic agents we used alcohol and ropivacaine. We mixed this with contrast agent to have a good visualization on control CT (third image). The outcome was highly positive—by the end of the procedure, the patient already reported a noticeable reduction in pain intensity. #ir #interventionaloncology #interventionalradiology #radiology #percutaneous #nerveblock #cancer

This is the case of a patient who presented with a unique liver metastasis. TACE was chosen as a treatment option. On the first try we chose a brachial access but were not able to pass the subclavian artery as it had an important kinking. The next day we chose a femoral access and managed to selectively catheterize the artery for segment 4 with a 2.4 microcatheter. For embolization we used doxorubicin and lipiodol. Cervical adenopathies in patient with history of breast cancer These were the only suspicious lesions at follow up and a percutaneous biopsy was suggested. As the lesions were superficial, it was easy to approach them with US guidance in safe conditions. Semi-automatic needle was used without trocar this time. #adenopathy #biopsy #percutaneous #interventionalradiology #interventionaloncology
