Magellan Health’s President of Behavioral Health and Chief Medical Officer Dr. Caroline Carney, MD, MSc, FAPA, FAPM, CPHQ shares her thoughts on a recently published study in JAMA highlighting the increase in a need for substance use disorder treatment for individuals 12-years-old and older, but a decline in those receiving that treatment.

A recent study published in JAMA reveals a troubling trend in behavioral health: individuals 12-years-old and older potentially needing treatment for alcohol use disorder (AUD), drug use disorder (DUD) and/or opioid use disorder (OUD) markedly grew between 2013 to 2023. Using data from the National Survey on Drug Use and Health, the researchers assumed that those meeting criteria for AUD, DUD or OUD needed treatment. Alarmingly, AUD rose from 6.6% to 10.2%; DUD increased from 2.6% to 9.6%; and OUD grew from 0.8% to 2.0%. According to the authors, “Despite increasing treatment needs, the percentage of participants receiving treatment decreased from 9.3% in 2013 to 6.5% in 2020. However, SUD treatment rates rebounded to 14.9% in 2022 and stabilized thereafter. AUD treatment decreased from 6.3% in 2013 to 4.3% in 2020. Similarly, DUD treatment declined from 13.4% in 2013 to 7.1% in 2020 but rebounded to 13.1% in 2022. OUD treatment decreased to 11.2% in 2020, rebounding to 22.1% in 2021 and decreasing to 18.3% in 2022." Among those who did not receive treatment, only 5.7% perceived a need for treatment, and even fewer (2.7%) perceived a need for treatment and made efforts in seeking treatment. The main reasons for not receiving treatment include reluctance to stop substance use, insufficient healthcare coverage, lack of awareness about treatment programs and negative impacts on employment and community perceptions. The perception of needing treatment is part of the condition of addiction. It is contingent on public health structures, providers, families and colleagues to support individuals in recognizing the need. Effective treatments should be found through their insurers and employee assistance programs. Finally, while stigma has declined since the pandemic, it remains alive for many age groups. We all have a role in health literacy and stigma reduction through public health messaging, supporting providers with the right data to drive effective interventions and providing families with materials on how to reach their loved one struggling with addiction. Organizations such as the Mayo Clinic and the Hazelden Betty Ford Foundation offer excellent primers to support families and loved ones in approaching the need for treatment. Ending on a positive note, last week the Centers for Disease Control and Prevention (CDC) reported that the age-adjusted rate of drug overdose deaths in the United States decreased 4.0% from 2022 (32.6 deaths per 100,000 standard population) to 31.3/100,000 in 2023. Action across many sectors and systems works. To see further decreases, more people need to recognize that they need help, help is available and treatment is effective.
