Understanding Root Cause Analysis: A Key to Problem Solving and Continuous Improvement In a fast paced world full of phrases like, “sweeping it under the rag”, “putting bandage on the problem”, and “wishing for it to go away” mindset, unattended and unresolved challenges and issues at organizations only get worse. In any organization, problems are inevitable. Whether it's a drop in productivity, a rise in employee turnover, or a decline in customer satisfaction, issues arise that can disrupt operations and impact overall success. The key to overcoming these challenges isn't just addressing the symptoms but identifying and tackling the underlying causes. This is where Root Cause Analysis (RCA) comes into play. What is Root Cause Analysis? Root Cause Analysis (RCA) is a systematic approach used to identify the underlying reasons for a problem or defect. Rather than merely addressing the immediate issue, RCA aims to uncover the root cause, which, when resolved, prevents the problem from recurring. This method is widely used across various industries and organizations including education, curriculum development, Instructional design, policies and programs, to name a few. It fosters a sense of unity, respect, and cooperation that benefits everyone involved. It is not just a desirable quality but a necessary foundation for success and well-being. Survey Report: It’s not what it looks Like When I was in public affairs, I frequently used and consistently stressed to my team about the importance of “what you see is what you get” (WYSIWYG) Pronounced (Wiz-ee-wig) when it came time to edit, create an image or simply print a photo. In a nutshell, it’s to replicate the original. I’m in a different profession and my use of WYSIWYG is not accurate in context of Culture Assessment or Survey. Generated report of responses after survey alone is not a representation of what an organization looks like, hence Follow-on assessment options such as focus group, random interviews, records and program review and or use of other tools are crucial to reveal and validate the report in other to help address issues and challenges while maintaining and improving what’s working. Example: The populated responses of survey results or pie chart (high or low scores) in categories such as hostile work environment, commitment, Leadership, Job Satisfaction etc.… does not tell the story until other tools are used to generate responses in the form of persons (demographics), environment, personal or professional challenges and or other categories. I heard and still hear the words “I hate this place” quite frequently and I always responded with “Help me understand what you hate about this place? The responses were broad, sometimes isolated, other times lingering issues and challenges at large, but most times perception of mass population. Not every decision (policy or operation) in an organization is popular, but Perception is reality for many and requires acknowledgement and address when these assessments are complete. Correct assessment is an inclusive cycle complete with plan of action for improvement and maintenance. Not a “one and done”

Do you know! : Why Employees Exit? There are numerous training for top leaders, and rightfully so, but only a fist full of Periodic or refresher Personal and professional development (PPD) training and or workshops are offered to First line Supervisors and Management. However, exit interviews show majority of employees leave organizations because of direct supervisors and management, followed by co-workers. High Percentage of the reasons for leaving are in areas of Equal of Employment Opportunity (Protected Classes), Equity, inclusion, accessibility, reasonable accommodation, Soft skills and lack of clarification in the company’s , culture Policies or Updates. Some of the challenges for those in direct supervision could either be lack of understanding, misinformation or just operating from outdated information. It is important to invest in your team to stay abreast with advocacy, compliance and awareness. (Yes PPD intertwine and can’t be separated). This will not only help improve retention and job satisfaction, it calibrates the team to stay on course with your agenda. Of the over 1,700 Plus and counting exit interviews I have conducted so far, only 3 employees left an organization because of top leaders. What other challenges do you see in workplaces that should be on the periodic refresher training or workshop? Visit mavisbrownconsulting.com : Your partner in People Excellence! For Growth & Upkeep Solutions.
