BY THE NUMBERS: As of Nov. 6, 373 hospitals across 139 systems in 39 states are currently approved for the Acute Hospital Care at Home (AHCAH) waiver program. We've updated our graph, depicting the steady growth of the Centers for Medicare & Medicaid Services' waiver. We support the extension of CMS' AHCAH waiver and federal commitment to increasing access to care in the home. We ask Congress to take urgent action before the end of the year to preserve long-term access to this critical care pathway. The everyday stories of hospitals working to accommodate the ever growing needs of patients in overcrowded emergency departments with no available beds on the upper floors is not new and for those patients and providers experiencing it firsthand, it is brutal. Adding the home as another site of care provides another point of access to care for patients and creates the opportunity for dynamic capacity for hospitals, filling a critical need. Medically Home is many things and tech company is only one of the ways to understand it. It created a model for hospital at home care, along with first-of-its-kind supporting technology and logistics capabilities, that made it possible to change the delivery of healthcare. The linked article from the AMA include some important of examples of the ways that health systems are reaching patients where they are, in their homes, improving care, improving access to care, addressing patients needs. Medically Home recognized from its beginnings that bringing acute care to the home, by nature changes the dynamic between a provider and a patient where they are working together, aided by insights that come from being with the patient in their own environment. Medication reconciliation is another example, but there are so many and they are powerful, reflecting that hospital at home care is changing the delivery of healthcare. Great to see more health systems and hospitals adding care for unhoused patients to their home hospital repertoire. Many Medically Home partners have worked within their patient communities to serve those experiencing homelessness and improve access to hospital care through the decentralized model. This morning, Medically Home announced it has partnered with SCP Health to enable virtual clinical command centers, a resource that can help resource-constrained hospitals offer hospital at home care to their patients. While many hospitals and health systems have applied for and been granted the waiver through CMS, some are not yet operating home hospitals. In Medically Home's experience, access to expert logistical, and technological support as well as resources like an established, staffed virtual command center, can remove the barriers and allow more patient access to this model of care.
