When natural disasters strike, patients with serious conditions like kidney disease and diabetes are especially vulnerable. Monogram’s Emergency Preparedness Program (EPP) is dedicated to providing critical support and continuity of care during these challenging times. When Hurricane Beryl hit, Yolanda faced disrupted dialysis access and power outages. Within 24 hours, Monogram secured her alternative dialysis care and provided ongoing support until her regular dialysis center reopened. 𝗛𝗼𝘄 𝗘𝗣𝗣 𝗠𝗮𝗸𝗲𝘀 𝗮 𝗗𝗶𝗳𝗳𝗲𝗿𝗲𝗻𝗰𝗲: • Proactive Outreach – We contact patients directly to check on their well-being and assess immediate needs. • Prioritizing At-Risk Patients – Patients needing life-saving treatments, like dialysis, receive our immediate focus or medications, like insulin, that are required to be taken daily • Delivering Solutions – From finding alternative care locations to filling life sustaining medications to supporting recovery with benefits like FEMA and SNAP, we’re here to help.
