  • 🏆 Celebrating Innovation in Patient Access! 🏆 We're thrilled to announce that Northwell Health’s Inpatient Discharge Referral Program has been recognized as a 2024 Best Practice by the Patient Access Collaborative! In this post, Research Analyst Ciannah Correa, MPH, explains how Holly Koehler, VP of Access, and Michele Pirog, Senior Director of Operations, developed a program that is revolutionizing patient care transitions. Rather than leaving discharged patients and caregivers to navigate complex follow-up appointments alone, Northwell Health's program provides structured support to coordinate post-discharge care. This initiative has achieved a remarkable 11% non-attendance rate—far below the industry average of 50%." Read the full blog post here: https://lnkd.in/eaQxTVmz Congratulations to the Patient Access Services team at Northwell Health for setting a new standard in patient-centered care! 👏 #PatientAccess #HealthcareInnovation #BestPractices #PatientCare #NorthwellHealth

Transforming Patient Access at Hyro I #1 Adaptive Communications Platform

Highlights from Patient Access Collaborative's recent webinar! Adrienne and Michael tackled a pressing issue in healthcare: the challenges faced by support teams grappling with high burnout and repetitive inquiries. They emphasized the importance of meaningful patient engagement and the ripple effects of agent turnover on costs and morale. The game-changer: Automation! 🤖✨By leveraging Hyro, healthcare systems transform how they handle patient interactions. One memorable quote from the session was, "we're shielding the staff from those routine interactions that can be offloaded to AI, improving access to care, enhancing the experience, and enabling scale without adding additional staff." Thank you Adrienne Woods and Hackensack Meridian Health for choosing Hyro to make such a great impact on patients and staff.
