Founded in 2006, the Primary Care Collaborative is a not-for-profit multi-stakeholder membership organization dedicated to advancing an effective and efficient health care system built on the strong foundation of primary care and the patient-centered medical home (PCMH). Our member are a broad group of public and private organizations, including payers, providers, non-profits and leading corporations dedicated to transforming health care.

Behavioral health care is long overdue for transformation through innovation. 🏥 Millions of people, particularly women and communities of color, face mental health and substance use challenges without adequate support. Women are disproportionately affected with higher rates of mental illness and higher rates of unmet behavioral health needs. The Innovation in Behavioral Health (IBH) model from Center for Medicare and Medicaid Innovation (CMMI) aims to improve access and health outcomes by integrating physical health and behavioral health services within trusted community-based behavioral health practices, offering a “no wrong door” approach to whole-person care. This model prioritizes health equity through screening for health related social needs and invests in building the health IT infrastructure.
