Ceramic fragment restorations are ideal as a minimally invasive procedure, avoiding unnecessary reduction of the healthy tooth structure. However, these restorations require a high degree of precision in terms of their indications and their fabrication. As such, there are four determining factors for this technique: ➡️ The color of the tooth to be restored must be the intended color, as the fragments cannot block darkened tooth structure, given their minimal thickness. ➡️ Fragments should only be fabricated in cases where the restoration or augmentation of dental structures is required. ➡️ The insertion axis of the fragments must be evaluated to verify if there is a need for removal or reduction of small retentive areas. ➡️ Their fabrication must be undertaken by a lab that has mastery of the technique. Cheers to 25 years of the tunnel approach! Did you know that IJPRD published the first landmark article on the tunnel approach back in 1999? The article described a new approach that aimed to improve esthetic results, reduce morbidity, and increase predictability when treating Miller Class I and II multiple adjacent recessions. The treatment may have been based on partial-thickness flaps, but it was the first to use the “tunnel” term we know and love today. When mucogingival deformities are present around dental implants, phenotype modification therapy can increase the peri-implant mucosal thickness and the amount of keratinized mucosa, improving the long-term clinical outcome. Free gingival grafts are considered the gold standard to increase keratinized mucosa, but they are less predictable on the lingual and often present with limited gains in tissue thickness due to a limited blood supply. The semilunar lingualized apically positioned flap (SLAP) with subperiosteal tunnel connective tissue graft (CTG) can increase both peri-implant mucosal thickness and keratinized mucosal width circumferentially. SLAP consists of one full-thickness, semilunar incision within the keratinized mucosa and buccal to the implant as well as two vertical releasing incisions mesial and distal to the implant, extending lingually into the alveolar mucosa. The tissue is apically positioned lingual to the implant and sutured in place, creating buccal space for the CTG via buccal subperiosteal tunneling. In issue 5 of the International Journal of Periodontics and Restorative Dentistry, Aziz Saqr et al demonstrate a case report using SLAP for peri-implant soft tissue phenotype modification around mandibular implants.
