January Image of the Month features Toxocariasis, which is a parasitic disease caused by the dog roundworm (Toxocara canis) or cat roundworm (Toxocara cati). Infection usually occurs by ingestion of infective eggs found in cat or dog feces. Humans are accidental hosts and the ocular disease, called ocular larva migrans, is unilateral in 90% of cases. The image here is of OLM manifesting as an old central posterior granuloma. In the acute phase the lesion would appear as a hazy white lesion with overlying vitritis. As pictured here, the inflammation has resolved and the resulting lesion is an elevated white mass with contraction and fibrosis of the retina.
