Choroidal Osteoma with Macular Neovascularization: A Rare Case Congratulations to Dr. Omar Mulki for presenting the June 2022 Case of the Month! This 40-year-old female experienced recent vision loss in her left eye, with visual acuity of counting fingers. Imaging revealed an amelanotic choroidal lesion occupying the macula, with a central blood spot. Swept-source OCT showed a thickened choroid, type 2 macular neovascularization (MNV), and exudative subretinal fluid. OCT angiography confirmed flow signals in the MNV, and ultrasonography identified the lesion as hyperreflective, consistent with a choroidal osteoma. Though an anti-VEGF injection was administered, the patient was lost to follow-up. Choroidal osteomas, benign calcified tumors often affecting women, require vigilant monitoring due to the risk of MNV, which can be treated successfully with anti-VEGF therapy.

Optic Nerve and Choroidal Coloboma in a Patient with Lifelong Vision Loss This case features a 66-year-old male with mild, lifelong decreased vision in his left eye, currently at 20/70. Optos imaging reveals an optic nerve coloboma with an adjacent choroidal coloboma, while Triton imaging shows optic nerve pits both temporally and nasally. Swept-source OCT highlights these pits in detail. Both optic nerve and choroidal colobomas result from incomplete closure of the embryonic fissure and may be unilateral or bilateral. Patients with choroidal colobomas face an increased lifetime risk of rhegmatogenous retinal detachment, while optic nerve pits are associated with serous macular detachment and macular neovascularization. This patient is under annual observation to monitor these risks.

Preoperative Imaging of a Large Giant Retinal Tear This case features preoperative Optos images showing a large giant retinal tear, a serious retinal condition that requires careful management to prevent further complications. Giant retinal tears can lead to complex retinal detachments, posing significant risks to vision. Early and detailed imaging, as seen in this case, is crucial for planning surgical intervention. Severe Bilateral Peripheral Ischemia and Neovascularization in a Patient with Diabetes and Sickle Trait This case highlights a patient with diabetes and sickle cell trait presenting with severe bilateral peripheral ischemia and neovascularization. The combination of diabetic retinopathy and sickle trait increases the risk of ischemic retinal complications, leading to proliferative changes. This case emphasizes the importance of vigilant monitoring and management in patients with overlapping systemic risk factors to prevent progressive vision loss. This case, submitted by Dr. Anand Temkar and Dr. Manish Nagpal, features a 35-year-old male who presented with one month of vision loss in his right eye. MultiColor imaging showed significant optic nerve swelling with multifocal, deep, white-centered retinal hemorrhages, while OCT revealed outer retinal edema extending from the nerve toward the macula. Extensive tests found no abnormalities except for severe microcytic anemia, with a hemoglobin level of 2.9 g/dl, likely due to iron deficiency from a poor diet. Anemia-related retinal issues typically involve retinal and sub-ILM hemorrhages, and severe anemia can rarely cause optic nerve edema. This case highlights the importance of considering anemia in patients with atypical optic nerve swelling or intracranial hypertension symptoms, though the reason for this patient's unilateral presentation remains unclear.
