    A few days ago, an explosion at a facility in Louisville, Kentucky destroyed the plant, killed 2 people, injured a dozen others, and sent chunks of steel into adjacent neighborhood and businesses. While the investigation is still ongoing, it will very likely surface that maintenance and/or operations practices will be at least partly at fault. It is way to easy to become complacent in an industrial environment. This was not a 190 foot tall, 2200 pound superheated steam boiler. This was a 20-ish foot tall "cooking vessel" used in the processing of food coloring. Often, management and operators alike lose focus of the basics of proper operations, training and maintenance as the day to day challenges of keeping things running and meeting production goals take the forefront. In 20+ years in critical operations, I can guarantee you this...this event came as the culmination of previous smaller "incidents" which in the Navy we called "near misses." I would bet that the investigation will reveal no less than three previous "close calls" that should have served as a warning. Loss production is the least of your worries as a supervisor. You have a responsibility to your employees and your community to operate safely. Regardless of who or if anyone is determined to be "at fault" in this incident, the facts are: - 2 people are dead - Community trust is destroyed - Production is halted - The company will be sued and fined - Someone, if not everyone, in the management team will be fired My condolences to the entire team, their families, and the community. Let this incident cause you to look internally at your organization and hopefully prevent a similar incident from a happening to you.
