Family owned and operated, Stebbins Hospice proudly serves patients in East Texas and surrounding counties. Our compassionate, caring and dedicated team provide individualized care to each patient as well as support for their families and loved ones. Electing hospice care does not mean that you’re giving up; it means you are choosing to focus on having the best possible life that you can for the rest of your life, however long that may be. We offer various hospice services to our patients, families, caregivers, and loved ones. A Registered Nurse Case Manager will coordinate all aspects of your care. Other members of the hospice team will work with you to assist with any needs you may have. The team members may include a Certified Nursing Aide, a dietician or RN to assess and perform a nutritional assessment and identify interventions needed for quality of life, a social worker to help with advanced care planning and medical social services, a Spiritual Advisor or Chaplain to provide spiritual support and encouragement, and a Medical Director to manage medication and physician services. Your Case Manager will also arrange for any medical equipment and supplies needed.
