Synapticure COO Joe Leinbach shared his insights and experience last week at the Innovation in Caregiving Conference on how Synapticure implemented the new Centers for Medicare & Medicaid Services Guiding an Improved Dementia Experience (GUIDE) program nationwide– and what other healthcare providers can do to improve dementia care. As Alzheimer’s and dementia diagnoses rise with an aging population, programs like GUIDE are more important than ever. Through CMS’ GUIDE program, Synapticure provides Medicare beneficiaries and their families with critical resources, including access to a specialized cognitive care team, a personal care coordinator, a 24/7 support line, and up to 80 hours of respite care for eligible patients. What is it like to be on the ground floor launching a new Medicare model that impacts millions of lives? We got a good window into that at the Innovation in Caregiving Conference last week. It was an honor to moderate a panel with leading clinicians and operators working on the CMS GUIDE model for families facing a dementia diagnosis. Kristen G. Schaefer, MD, HMDC, FAAHPM at Care Dimensions, SUMMER Cavender-WYSONG at ARKANSAS PALLIATIVE CARE, LLC, and Joe Leinbach at Synapticure have had the program up for 7 months, and Vanessa Guzman at JVS Health will be launching this July. We are grateful to them for sharing their experience with us. Learnings: ✨ Values Alignment: Providers were drawn to apply to be included in the model because of strong alignment with their organizations—focus on family caregivers, health equity, and innovation. 📈 Strong Consumer Demand: People are actively googling to find GUIDE providers! Once connected, consumers are thrilled—many can’t believe they’re able to access such expansive GUIDE services for free. 📢 Need for Awareness: With over 7 million families who could benefit, there’s still a big need for consumer and provider education. It often takes multiple attempts to engage families for comprehensive assessments, and many referring providers aren’t yet aware of the program’s potential. ⚙️ Key Operational Challenges: - Finding respite providers at mandated rates and getting them CMS-approved - Streamlining the comprehensive assessment process - Navigating administrative portal issues 💡 Going Beyond the Basics: Some providers are offering services above and beyond what’s required. Care Dimensions is providing palliative care consultations, and ARKANSAS PALLIATIVE CARE, LLC is exploring therapy services for caregivers. We’re excited to see how these additions impact outcomes. 🚀 Scaling for Growth: Established providers are building infrastructure to scale—enhancing technology for smoother assessments and partnering with organizations like ianacare to expand capacity and capability. (We are excited to be working with Care Dimensions and JVS Health!) 😔 Coverage Limitations: It’s “really sad” that only patients with traditional Medicare can access these benefits for now. We’re all looking forward to working with Medicare Advantage plans to roll out similar programs. Beyond the panel, it was energizing to connect in person with so many like-minded colleagues and partners who are leading this charge to effectively empower family caregivers within care delivery. Special thanks to conference organizers Dhruv Vasishtha and Kristen Speridakos, as well as other panelists and moderators including Jessica Nam Kim, Barry Jacobs, Ian Chiang, and Sandra Clark, MPH. This conference planted a seed 7 years ago and it’s wonderful to see how the industry has grown. We look forward to setting up more frequent dialogues, and this becoming an annual event!
