Health care organizations across the U.S. are increasingly partnering with local faith leaders to strengthen community connections and bridge care gaps for health conditions, including serious illness. Faith leaders, as trusted members of their communities, are well positioned to engage with parishioners and residents who may distrust the health care system or feel uncomfortable accessing care. The Better Care Playbook recently spoke with Elder Angela Overton, Senior Advisor and Director of Faith Community Partnerships at The Coalition to Transform Advanced Care (C-TAC), to learn about the Louisville Community Model of Care Project. The project aims to improve the quality of life for Black community members with serious illness by providing access to care and investing in trusted partners to address disparities in access to serious illness care. It was great to attend the The Coalition to Transform Advanced Care (C-TAC) 2024 National Leadership Summit in Washington, DC, where The Sheri and Les Biller Family Foundation was proud to be a sponsor. Meeting with partners and leaders committed to advancing policy related to health equity and also patient-centered models of care was very uplifting. I loved the hallway conversation with my colleagues Rachael Watman, Vice President of Programs at The Rita and Alex Hillman Foundation, and Rani Snyder, Vice President of Programs at The John A. In Washington, DC and around the country we have heard a range of reactions to the election results from pollsters, pundits, Capitol Hill staff, advocates, citizens, and many more. At C-TAC, we are familiar with changing political tides and with collaborating in a bipartisan, bicameral fashion regardless of who holds the majority or lives in the White House. We have worked successfully with both political parties and Independents and multiple Administrations to improve the quality of life for individuals with serious illness and their families. In concert with other like-minded organizations, we will work to ensure that funding levels, opportunities for innovation, and program integrity and efficiency reflect our values, priorities, and the needs of those with serious illnesses. Our work to improve payment models, access to advance care planning, the Medicare hospice program, home and community-based services, and care equity will continue. C-TAC pursues a comprehensive policy agenda with the goal of ensuring that all Americans living with advanced or serious illness receive comprehensive, high-quality, person- and family-centered care that is consistent with their goals and values and honors their dignity. The The Coalition to Transform Advanced Care (C-TAC) does such critical work bringing together all the stakeholders to transform how we care for advanced illnesses, and works tirelessly to advance policy toward that goal. Given the silos we have in our healthcare system, C-TAC serves as a critical bridge between government at all levels and healthcare providers across the entire spectrum. We gleaned so many insights from the C-TAC Annual Leadership Conference last week. • Red or blue, policy matters, and behind the partisanship we see on TV, there are many behind the scenes on both sides of the aisle working to advance healthcare policy, from the expansive Olders American Act which brings us programs like Wheels on Wheels to state-level Caring for Caregivers Act for tax relief. • As we move toward comprehensive care models with interdisciplinary teams, staffing challenges will compound. It is important to optimize staffing by ensuring clinicians are doing above the license work, and other elements can be delivered by non-clinical staff or best of breed partners. • Measurement is a priority. Centers for Medicare & Medicaid Services is prioritizing collecting data in many ways for the GUIDE model. C-TAC is doing great work with TFA Analytics and state government on the Act Index to measure overall performance for advanced illness. • Programs such as GUIDE can only do what the statute allows. Again, policy matters. To that point, it was great to hear that Liz F. built on top of what Adam Boehler built and we are hopeful for continuity with the upcoming election regardless of outcome. It was so meaningful to meet many changemakers and clinicians doing important work for serious illness, including Anna Chon, Kristen G. Schaefer, MD, HMDC, FAAHPM, Heidi Young, MD, Stacey Downey, Lindee Goh, Emily Cane, Lea Tessitore, MSB, MBA, Sandra Clark, MPH, Erika Diaz Narvaez, Diane Ty, Cal Cates, Tonya Saffer, Laura Makaroff, Melissa Singleton, and Leigh Ann Kingsbury. INELDA attended the The Coalition to Transform Advanced Care (C-TAC) National Leadership Summer this week, a conference to connect changemakers working to improve quality of life for people living with serious illness, and their caregivers. As a member of C-TAC, we lend our voice to advocate for the importance of advance care planning, doula integration in the health care system, and increased access to social workers through Medicare billing, and we learn what other partner organizations are researching in the serious illness space. This organization and annual conference has been instrumental to our growth as an organization, both by showcasing dynamic and cutting-edge speakers and by the support it has offered INELDA through quarterly check-ins on our own policy and partner interests. It was an honor to share my story at The Coalition to Transform Advanced Care (C-TAC) Annual Summit in Washington, D.C. As a person living in remission for angiosarcoma, a rare, incurable cancer, and serving as a remote caregiver for my beloved Mom, I am grateful for the power of connection that advocates share in magnifying the voices of patients and caregivers for critical policy change. In these uncertain times before key elections, this week showcased the promising bipartisan work that we can and must continue to drive the solutions that all patients and caregivers need. Our shared stories bind and galvanize us, fostering progress and unity. Proud to be a 20-year staff member at the American Heart Association and the State Volunteer President for AARP Colorado, both founding members of C-TAC. Together, we can move mountains and make a significant impact on healthcare policy.
