Last week, Unite Us’ Allison Kelly had the incredible opportunity to participate in The Great American Teach-In (#GATI)✏️📚. She joined the enthusiastic Leopards of Lutz K-8 for a day of fun and learning! 💫The Great American Teach-In is an event that allows students to explore a world of possibilities, exposing them to new careers, hobbies, and activities they might not have known existed. Every year thousands of volunteers in #HillsboroughCounty share their unique stories and experiences with students of all ages. 💬 “𝙏𝙝𝙚𝙨𝙚 𝙇𝙚𝙤𝙥𝙖𝙧𝙙𝙨 𝙨𝙝𝙤𝙪𝙡𝙙 𝙣𝙚𝙫𝙚𝙧 𝙝𝙞𝙙𝙚 𝙩𝙝𝙚𝙞𝙧 𝙨𝙥𝙤𝙩𝙨– 𝙩𝙝𝙚𝙮 𝙖𝙧𝙚 𝙚𝙣𝙩𝙝𝙪𝙨𝙞𝙖𝙨𝙩𝙞𝙘, 𝙠𝙞𝙣𝙙, 𝙖𝙣𝙙 𝙨𝙤 𝙫𝙚𝙧𝙮 𝙗𝙧𝙞𝙜𝙝𝙩! Recently we announced the launch of our enhanced Social Needs System™! ⚡This powerful tool helps organizations identify, address, and measure #socialneeds for the people they serve in their communities. As #healthcare becomes more integrated, addressing both medical and social needs is crucial.🏥 But how do we accurately measure social vulnerability to target resources effectively? 🤔 𝐖𝐡𝐚𝐭 𝐢𝐬 𝐭𝐡𝐞 𝐒𝐨𝐜𝐢𝐚𝐥 𝐍𝐞𝐞𝐝𝐬 𝐒𝐲𝐬𝐭𝐞𝐦 (𝐒𝐍𝐒™): Created by #UniteUS SNS™ is a 𝐩𝐫𝐨𝐩𝐫𝐢𝐞𝐭𝐚𝐫𝐲 framework that measures and addresses Drivers of Health (#DoH). SNS™ assesses individuals' social needs across 12 factors, including food security, childcare, housing, employment, and more, to understand their impact on health and well-being. Alternative tools that measure different aspects of social drivers and vulnerabilities like the Social Vulnerability Index (#SVI) and Area Deprivation Index (#ADI) assess social, and economic challenges from a broader perspective. We are thrilled to see our new peer-reviewed article published today in the 𝐉𝐨𝐮𝐫𝐧𝐚𝐥 𝐨𝐟 𝐂𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲 𝐇𝐞𝐚𝐥𝐭𝐡 𝐍𝐮𝐫𝐬𝐢𝐧𝐠 demonstrating the impact of social care referral navigation on the mental health and well-being of young families. Here are some key takeaways: ✅ With support from trusted navigators, parents report more hope and confidence in addressing their children’s needs. Through technology-enabled social care coordination powered by Unite Us, parents report relief from the burden of self-navigating community based resources. ✅ Our model of care can improve access to supportive community services for young families with social needs and increase parental protective factors known to improve short and long-term outcomes for families. ✅ This evaluation of First 1,000 Days Suncoast’s Family Navigation program demonstrated statistically significant improvements in parental social support, an additional four healthy mental health days per month, and significant improvements in perceived stress, among parents receiving navigation support. We’re encouraged by these results and look forward to continuing this important research. We want to extend a special thanks to our First 1,000 Days Suncoast partners Chelsea Arnold, DNP, APRN, FNP-bc and Mary O’Connor, as well as acknowledge the contributions of our Unite Us Research and Evaluation team including Amanda Terry, PhD, MPH, MA, Zackery White, PhD MPH , Gillian Feldmeth, and Halima Ahmadi-Montecalvo, PhD, MPH. Our incredible 𝐒𝐚𝐥𝐞𝐬 𝐓𝐞𝐚𝐦 𝐖𝐞𝐬𝐭 just wrapped up their annual retreat in beautiful Portland, Oregon. 🌳 It was a time well spent filled with reflection, collaboration, and plenty of laughter! 🗨️ “This week the Unite Us Sales Team West got together for their annual retreat in Portland, Oregon,” says Jessica Ohlssen, Regional VP of Sales, West. “They accomplished their goal of reviewing 2024 successes and plans for furthering Unite Us into communities addressing #socialdriversofhealth.” 𝐂𝐨𝐥𝐥𝐚𝐛𝐨𝐫𝐚𝐭𝐢𝐨𝐧 𝐢𝐬 𝐊𝐞𝐲 𝐟𝐨𝐫 𝐃𝐫𝐢𝐯𝐢𝐧𝐠 𝐂𝐡𝐚𝐧𝐠𝐞 🗝️ 💭As Taylor Justice, our Co-Founder and President put it, “Collaboration is critically important. When you bring together three different sectors—#government, #healthcare, or #community—each organization needs to partner on how to provide care to individuals in communities. It’s not just a government issue, it’s not just a healthcare issue, it’s not just a community issue. 𝐈𝐭’𝐬 𝐚𝐛𝐨𝐮𝐭 𝐞𝐯𝐞𝐫𝐲𝐛𝐨𝐝𝐲 𝐜𝐨𝐦𝐢𝐧𝐠 𝐭𝐨𝐠𝐞𝐭𝐡𝐞𝐫.” And that's exactly what our 𝐒𝐚𝐥𝐞𝐬 𝐓𝐞𝐚𝐦 𝐖𝐞𝐬𝐭 did! Reflecting on 2024 and strategizing for 2025, they focused on ways to further our mission and make Unite Us even more accessible and collaborative for all stakeholders across the care continuum. During this year’s retreat, to foster collaboration and strategic thinking 💭 our Sales Team met at the Kiln Coworking space. Evening dinners 🍽️ at Bar Casa Vale in Portland provided an opportunity for team bonding. 📸 Cade McCallum (Sales Executive), J.R. When it comes to #socialcare, knowing you need help is often the first step—figuring out how to get help is second. 📄 Public resource directories provide helpful lists of social care services and organizations — but also put the onus on individuals to navigate complicated systems on their own. The key🔑 to social care transformation is making sure people are not just pointed toward resources, but are actually connected to the services they need. That’s where closed-loop referral systems come in.🔄 ⭐ Unite Us' closed-loop referral platform tracks every referral from start to finish, ensuring individuals receive coordinated, informed, hands-on support throughout their entire care journey. Plus, providers can follow up and chat in real time, ensuring the connection is made and that care continues until all needs are met. By bridging the gap between identifying services and accessing them, we remove the burden from individuals who are often overwhelmed by the complexities of the #healthcare and #socialservice landscapes. Because it’s often not just about locating the right help—it’s about getting the coordinated support you deserve, every step of the way. 💡 Learn more about how we’re revolutionizing care coordination: https://bit.ly/46djsQK

This #𝐍𝐚𝐭𝐢𝐨𝐧𝐚𝐥𝐑𝐮𝐫𝐚𝐥𝐇𝐞𝐚𝐥𝐭𝐡𝐃𝐚𝐲 🩺, we are focusing on 𝐀𝐝𝐯𝐚𝐧𝐜𝐢𝐧𝐠 𝐌𝐚𝐭𝐞𝐫𝐧𝐚𝐥 𝐇𝐞𝐚𝐥𝐭𝐡 𝐢𝐧 𝐑𝐮𝐫𝐚𝐥 𝐂𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐢𝐞𝐬. According to the National Organization of State Offices of Rural Health (NOSORH), more than 61 million Americans live in rural communities. However, #rural areas face unique challenges when accessing healthcare. Our recent webinar, '𝐅𝐫𝐨𝐦 𝐃𝐚𝐭𝐚 𝐭𝐨 𝐈𝐦𝐩𝐚𝐜𝐭: 𝐌𝐞𝐚𝐬𝐮𝐫𝐢𝐧𝐠 𝐒𝐮𝐜𝐜𝐞𝐬𝐬 𝐖𝐡𝐞𝐧 𝐈𝐧𝐭𝐞𝐠𝐫𝐚𝐭𝐢𝐧𝐠 𝐒𝐨𝐜𝐢𝐚𝐥 𝐂𝐚𝐫𝐞 𝐢𝐧𝐭𝐨 𝐇𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞,' featured experts from Sarasota Memorial Health Care System who discussed a few of these challenges — and the value of strong #partnerships in overcoming them. 💪 🗨️ 𝐌𝐚𝐫𝐲 𝐎' 𝐂𝐨𝐧𝐧𝐨𝐫, 𝐌𝐒𝐍, 𝐑𝐍, 𝐂𝐒𝐒𝐆𝐁, 𝐌𝐚𝐧𝐚𝐠𝐞𝐫 𝐇𝐞𝐚𝐥𝐭𝐡 𝐂𝐚𝐫𝐞 𝐄𝐪𝐮𝐢𝐭𝐲, shared, "In smaller areas, resources can be scarce, making it difficult to support #𝐦𝐚𝐭𝐞𝐫𝐧𝐚𝐥𝐡𝐞𝐚𝐥𝐭𝐡." Data collection and organizational efforts can be particularly challenging, especially when considering new mandates. 🗨️ Chelsea Arnold, DNP, APRN, FNP-bc, 𝐈𝐧𝐢𝐭𝐢𝐚𝐭𝐢𝐯𝐞 𝐌𝐚𝐧𝐚𝐠𝐞𝐫 for First 1,000 Days Suncoast, highlighted the power of partnerships: "Leaning on #communitypartners is crucial. By bringing that #data to local foundations and sharing that information with other community partner organizations, they may decide that they want to expand to help that area or that rural community." 𝐍𝐚𝐭𝐢𝐨𝐧𝐚𝐥 𝐑𝐮𝐫𝐚𝐥 𝐇𝐞𝐚𝐥𝐭𝐡 𝐃𝐚𝐲 gives us an opportunity to honor the incredible efforts of rural healthcare providers, communities, organizations, and other stakeholders dedicated to addressing the unique healthcare needs of #ruralAmerica. 💡#Didyouknow that 32+ states recommend or require social needs screening? Addressing social needs is crucial for improving health equity and delivering whole-person care. 𝐖𝐡𝐚𝐭 𝐢𝐬 𝐖𝐡𝐨𝐥𝐞-𝐏𝐞𝐫𝐬𝐨𝐧 𝐂𝐚𝐫𝐞? Whole-person care recognizes the crucial role that social drivers of health (#SDOH) play in a person’s overall well-being and quality of life. Unmet social needs such as food, housing, transportation, and social connection significantly contribute to health disparities, especially among vulnerable populations. As the healthcare industry shifts towards value-based payment models and a more integrated healthcare approach, there’s a growing emphasis on addressing these social needs to improve health equity and deliver whole-person care. 𝐖𝐡𝐲 𝐢𝐬 𝐖𝐡𝐨𝐥𝐞-𝐏𝐞𝐫𝐬𝐨𝐧 𝐂𝐚𝐫𝐞 𝐈𝐦𝐩𝐨𝐫𝐭𝐚𝐧𝐭? ⬇️ Addressing social needs can lead to better health outcomes, such as reduced hospitalizations 🏥 and emergency department visits. 👇💲By preventing health problems and addressing underlying social issues, whole-person care can reduce healthcare costs. 🙂 Patients who receive whole-person care often report higher levels of satisfaction with their care.
