A public foundation improving well-being in Arizona by addressing root causes & broader issues that affect health.

About us

Vitalyst Health Foundation has three goals: 1) Collaborative Networks working to improve the conditions that impact health. 2) Strong Community Partners equipped to advance health equity. 3) Informed Decision-Makers prioritizing community health and well-being. Our goals span across four priorities: Civic Health - Supporting long-term efforts that improve active community participation. Food Systems – Strengthening food systems across rural and urban areas in Arizona. Health Care Integration – Supporting the integration of physical, mental and social determinants of health. Housing – Supporting statewide housing initiatives along the spectrum of housing. Vitalyst Health Foundation has been an Arizona nonprofit corporation since 1907. For most of its existence, the nonprofit operated one or more hospitals in the Phoenix metropolitan area, including St. Luke’s Medical Center and St. Luke’s Behavioral Health Center in Phoenix, Arizona and Tempe St. Luke’s Hospital in Tempe, Arizona. In 1995 all hospital facilities were sold to OrNda HealthCorp and the nonprofit converted to an independent health foundation known initially as St. Luke’s Charitable Health Trust (1996-2000), then as St. Luke’s Health Initiatives (2000-2016) before changing its name in March 2016 to Vitalyst Health Foundation.

  • Creating a healthier Arizona, together 🌵 By strengthening Civic Health, Housing, and Health Care Integration, we’re focusing on areas to create a thriving Arizona. Here’s how these three areas build a healthier community: ✔️ When people engage in community activities like voting, volunteering, and supporting local projects, they create stronger, more resilient neighborhoods—this is Civic Health. ✔️Stable and affordable housing is the foundation of well-being, especially in a challenging market. Many families face tough choices between housing and essentials like food and healthcare. ✔️ Health includes physical, mental, and social aspects working together. Integrated care connects these areas, making it easier for people to access comprehensive, coordinated support. Together, these areas form the foundation for a stronger, healthier Arizona where everyone has a fair chance at well-being.

  • 🇺🇸 Honoring those who have served and continue to serve our country. And we’d like to take this moment to recognize an organization that supports veterans and their families year-round: the Arizona Coalition for Military Families. They go beyond traditional support by addressing critical needs like mental health, employment, housing stability, and family support, tackling many issues impacting our military community. If you or someone you know could benefit from their assistance, reach out to see how they can help!

  • Meet The COCONINO COALITION FOR CHILDREN & YOUTH, a 2023 recipient of last year’s System Change Grant! 🌟 Their work is making a lasting impact by: 1. Collaborating with others: They work alongside various organizations, joining forces for a shared purpose that brings diverse strengths to the table. 2. Targeting a specific health outcome: Their focus is on creating self-healing communities through trauma-informed systems that acknowledge individual experiences to improve northern Arizona communities. 3. Using a systems change approach: They tackle the root causes of inequitable systems and racial inequities by developing, in conjunction with the City of Flagstaff and the County, a mental model change ensuring resilience information is understood at City and County levels and enacted as policy change. 4. Prioritizing health equity: They ensure that everyone they serve has fair and equitable treatment. Want to drive systems change in your community? Apply for this year’s grant and join the movement for a healthier Arizona! https://lnkd.in/gP7cJuPy
