Patients tell us they’re thankful too.     Value-based primary care requires caring more between in-office appointments. That’s why patients with multiple chronic conditions or complex social determinants of health are assigned a dedicated RN.    This added layer of support results in fewer hospitalizations and $3,900 in annual cost savings per patient.     Patients never see a bill for this service. So instead, in hundreds of different ways, they tell our clinical team, “thank you.”

    I always enjoy participating in panel discussions, particularly with exceptional participants and exciting topics. For example, an event this week reinforced how critical patient trust is in primary care. Dr. Veronica Mallett leads a partnership between Morehouse School of Medicine and Common Spirit, the More in Common Alliance, focused on diversifying the healthcare workforce. They’re placing residency sites in diverse communities like Bakersfield, California, with the hope that students in those communities will go off to med school in Atlanta but come back to practice medicine in their hometowns. A familiar face makes provider trust come that much easier. Jillian P. Wright is using the trust in PCPs to boost mammogram compliance. Onsite Women’s Health partners with PCPs to offer imaging services directly in the PCP office. Rather than requiring a separate trip to get a mammogram, the patient can complete her mammogram during her PCP visit – such a great service for women who frequently juggle work, household and caregiver responsibilities. Onsite Women’s Health has seen mammogram compliance increase from 50% to as high as 90% with their partner clinics. That’s unheard of — a testament to the strength of the PCP-patient relationship. I found myself sharing several patient stories that highlight the power of patient trust. Like one patient who had been to the ER 72 times over the course of a year, usually for avoidable reasons. Our partner physician was unaware until Wellvana presented the data. A plan was developed, and now the patient has standing appointments every week, just to check in on whatever is bothering them. The patient is getting better care at a lower cost, thanks to that trusting relationship. Crosspollination is powerful.   • An American Heart Association study interviewed patients and clinicians in Accountable Care Organizations and found: • Better patient care and outcomes • Improved experience for clinicians • Smarter technology and performance tracking “We see these life-changing impacts up close every day," says Wellvana CEO Kyle Wailes.     It’s not value-based care if the patient doesn’t win first. And you don’t just have to take my word for it. The American Heart Association is out with a fresh study that offers some independent data on the clinical benefits of participating in an Accountable Care Organization. It’s a small study, but it confirms much of what we see at Wellvana. 🩺 In these survey-style interviews, patients “overwhelmingly” reported better care than they received under fee-for-service medicine. They noted stronger ties to their primary care provider, better care coordination and a team that included care managers and social workers. Many received help with transportation, food, housing and financial assistance. Those are the basics for any high-performing ACO. 👩⚕️ It wasn’t just patients singing praises. Physicians and other clinicians also agreed that being part of an ACO enabled them to care for complex patients in a more holistic way. Because when you’re on the hook for all the costs surrounding a patient’s care, multidisciplinary teams become a must. And what’s good for the patient is also good for their providers. 📲 One more thing. Practices benefited from the technology support from an ACO. Primary care providers are often flying blind related to what a patient is doing outside of their office. But once they know a patient has been to the ER too many times in the past year, often for avoidable reasons, they can do something about it. We see these life-changing impacts up close every day. So it’s especially rewarding when others affirm that accountable care can be a rising tide where everybody wins.

He would get calls from geriatricians who almost always had the same request: Can you get this patient off at least one of their meds?    The National Institutes of Health says the regular use of five medications counts as polypharmacy. But it’s very common for patients with multiple chronic conditions to have a dozen or more meds. Some become unnecessary.   • 🛼 For the next 53 days, seniors are making big decisions about their healthcare. It's all-skate. The Annual Open Enrollment Period allows anyone 65 and over to start a new Part D prescription plan, Medigap or Medicare Advantage plan. They can also switch MA plans or return to Original Medicare. But most don’t. A KFF analysis published this year found 7 in 10 Medicare beneficiaries did not compare their coverage, even though benefits and networks change each year. (link in comments) 🌊 It can seem like a tidal wave of information. It's complicated, which is why we’ve boiled it down in our 2024 Medicare Review, fresh with checklists and room for notes. (At least make sure the doctors you’ve seen in 2024 will honor your plan in 2025.) Share with (or tag) anyone who is approaching or beyond their Medicare birthday. As a value-based care enabler, Wellvana supports patients with all types of coverage, and those patients never see a bill for our help between appointments. But we know how important the right plan or combination of plans can be. Just like taking a 30-minute walk, meditating, scheduling annual screenings and your wellness visit, it’s good for your health – and your wallet – to take a yearly look at your coverage.

    🫡 On Primary Care Week, we salute the heroes of value-based care — Primary Care physicians and health professionals.     🤔 Think about this: Primary Care benefits everyone, everywhere. And the more you visit your PCP and engage with their team, the lower your total cost of care will be in the long run.  According to a 2022 NIH study, the effect of primary care visits on total patient care cost is even more stark for patients with multiple chronic conditions.
