They partner with safety-net primary care providers and act as extension of the clinical staff to provide comprehensive clinical and mental health care while addressing the many social barriers to achieving a high quality of life such as access to housing, food, or transportation.   • We are excited to announce our transition to a Public Benefit Corporation® (PBC), reinforcing our mission to connect underserved communities to high-quality care. This change underscores our commitment to social impact and ensuring equal access to transformative, AI-enabled healthcare. Our innovative care model has effectively engaged patients lacking preventative care, reducing emergency department visits by 52% and hospital admissions by 39%. This has resulted in over $25,000 in annual savings for the highest-risk Medicaid and Medicare beneficiaries while dramatically improving their quality of life. We have also facilitated more than $10 million in community payments through our enablement model and local hiring – and aim to expand this impact by facilitating $30 million in community payments in 2025. This work is made possible by the unwavering dedication of our entire Pair Team. Their passion and perseverance propel our mission forward, ensuring that patients who might otherwise be overlooked receive the high-quality care they deserve.     🎉 What a year it has been at Pair Team! As we wrap up 2024, we’re reflecting on the incredible milestones our team has achieved together with our partners and patients, including: ✅ 5k+ individuals enrolled in our programs this year. ✅ 50k+ care plans created to support whole-person care. ✅ 20k+ care plans completed over the last 12 months. ✅ HbA1c screenings increased by 236% for our enrolled patients—helping us take a significant step toward better health outcomes. ✅ Over $500k distributed to Community-Based Organizations across California, ensuring resources reach the communities that need them most. We couldn’t have done it without the support and dedication of our amazing team, partners, and the individuals who trust us to support their health and well-being.     IEHP Foundation is excited to award $2.3 million in grants to enhance the organizational infrastructure of local nonprofits. 🎉 This funding supports the Inaugural Leadership Network, a two-year leadership development program designed for community-based organizations (CBOs) in the Inland Empire that focus on the social determinants of health. Over the next two years, IEHP Foundation’s Leadership Network will invest a total of $4.6 million in direct grant funding to 40 CBOs serving families with children across San Bernardino and Riverside counties. Depending on the size of their annual operating budget, selected organizations will receive a total of $50,000 or $130,000. Each organization nominated an established executive leader, such as a CEO or executive director, and an emerging leader, both of whom will participate in the Leadership Network throughout 2025 and 2026. The 80 leaders will convene quarterly to bolster their skills in business management, communication, strategic thinking, change leadership, team building, succession planning and public policy and advocacy.
