Key Qualifications
- Coordinate, monitor, and adjust all aspects of a client's care from intake to discharge.
- Ensure that everyone who delivers care to someone is working toward a common goal.
- Communicate with members of the team and review client health information.
- Screen and assess eligible patients.
- Ensure that a client is engaged with necessary clinical and/or wrap-around services in the community, helps transitions happen smoothly, and ensures that the work of multiple caregivers is coordinated.
- Evaluate the treatment plan, and facilitate the services needed to ensure all therapies are complete.
- Coordinate with justice system stakeholders and community partners to ensure the best healthcare outcomes for clients.
- Advocate for patients’ rights and ensure that they receive appropriate care.
- Conduct assessments, set goals, coordinate care, manage crises, and drive process improvements.
- Work closely with a dynamic team to resolve conflicts and achieve common goals.
- Establish and maintain strong working relationships with peers, partners, and clients.
- Provide regular status updates, summarizing key issues and required actions.

At Janus, base salary is one part of our total compensation package and is determined within a range. This provides the opportunity to progress as you grow and develop within a role. The base pay range for this role is between $25.00 and $28.00 per hour and your base pay will be considered on your job-related skills, training, qualifications, experience, and relevant education.

Requirements
Experience:
- 2+ years of full-time case management in substance use disorder treatment or integrated behavioral health programs (considered equivalent to a bachelor’s degree).
- Bachelor’s degree in a behavioral health-related field with some case management or care coordination experience.
- Treatment of substance abuse disorders.
- Co-occurring health and mental health problems.
- Familiarity with electronic health record systems like AVATAR.
- Application of SAMHSA’s best practice indicators.
- Drug and alcohol testing to monitor treatment plans and medical instructions.
- Education in Social Work (Preferred)
- Registered or Certified by CADTP or CCAPP (Preferred)
- MFTi or ASW interns are encouraged to apply.
- Experience in peer support modalities (e.g., WRAP, 12-step recovery).
- Knowledge of HIPAA standards and CFR42 confidentiality restrictions.
- Current or recent professional counseling experience (preferred).
- Excellent written and oral communication.
- Exceptional interpersonal skills and a compassionate nature.
- Bilingual - Spanish strongly preferred.
- Proficiency in any of the following platforms is preferred: Microsoft 365, Google Workspace, Apple iWork, or LibreOffice.
- Strong understanding of cloud-based and standalone office suite applications.
- Strong understanding of electronic health record (eHR) systems.
- Ability to troubleshoot and resolve technical issues related to office suites.
- Possess a California driver’s license, ability to pass a DMV check, and drive clients in a personal vehicle.
- Bilingual (preferred).
