Key Qualifications

• Coordinate, monitor, and adjust all aspects of a client's care from intake to discharge.
• Ensure that everyone who delivers care to someone is working toward a common goal.
• Communicate with members of the team and review client health information.
• Screen and assess eligible patients.
• Ensure that a client is engaged with necessary clinical and/or wrap-around services in the community, helps transitions happen smoothly, and ensures that the work of multiple caregivers is coordinated.
• Evaluate the treatment plan, and facilitate the services needed to ensure all therapies are complete.
• Coordinate with justice system stakeholders and community partners to ensure the best healthcare outcomes for clients.
• Advocate for patients’ rights and ensure that they receive appropriate care.
• Conduct assessments, set goals, coordinate care, manage crises, and drive process improvements.
• Work closely with a dynamic team to resolve conflicts and achieve common goals.
• Establish and maintain strong working relationships with peers, partners, and clients.
• Provide regular status updates, summarizing key issues and required actions.

Pay & Benefits

At Janus, base salary is one part of our total compensation package and is determined within a range. This provides the opportunity to progress as you grow and develop within a role. The base pay range for this role is between $25.00 and $28.00 per hour and your base pay will be considered on your job-related skills, training, qualifications, experience, and relevant education.

As part of our team, you'll frequently move equipment weighing up to 25 pounds, ensuring everything is in the right place for our various needs. You'll also have the opportunity to stay active by occasionally moving around the office to access file cabinets, office machinery, and more.

Requirements

Experience:

• 2+ years of full-time case management in substance use disorder treatment or integrated behavioral health programs (considered equivalent to a bachelor’s degree).
• Bachelor’s degree in a behavioral health-related field with some case management or care coordination experience.
• Treatment of substance abuse disorders.
• Co-occurring health and mental health problems.
• Familiarity with electronic health record systems like AVATAR.
• Application of SAMHSA’s best practice indicators.
• Drug and alcohol testing to monitor treatment plans and medical instructions.
  
  
  

Certifications:

• Education in Social Work (Preferred)
• Registered or Certified by CADTP or CCAPP (Preferred)
• MFTi or ASW interns are encouraged to apply.
  
  
  

Skills & Knowledge:

• Experience in peer support modalities (e.g., WRAP, 12-step recovery).
• Knowledge of HIPAA standards and CFR42 confidentiality restrictions.
• Current or recent professional counseling experience (preferred).
• Excellent written and oral communication.
• Exceptional interpersonal skills and a compassionate nature.
• Bilingual - Spanish strongly preferred.
• Proficiency in any of the following platforms is preferred: Microsoft 365, Google Workspace, Apple iWork, or LibreOffice.
• Strong understanding of cloud-based and standalone office suite applications.
• Strong understanding of electronic health record (eHR) systems.
• Ability to troubleshoot and resolve technical issues related to office suites.
  
  
  

Other Requirements:

• Possess a California driver’s license, ability to pass a DMV check, and drive clients in a personal vehicle.
• Bilingual (preferred).
