Starting Salary $100k-$119K per year + annual bonuses up to $5,000

Schedule Monday to Friday, No Weekends, No Calls

Hours Flexible, 9 am-5 pm or 7 am-3 pm

Locations Brooklyn, Manhattan, and Queens

Equipment Provided Laptop and Wifi

Job Details

The Home Care RN Case Manager sees patients in their homes.

What you will be doing

• Provide case management services for homecare patients
• Complete initial assessments, OASIS, recertifications
• Creates and implements a plan of care
• Collaborates with the interdisciplinary team and coordinates patient care
• Educates patients and families
  
  

Qualifications

• Must have an active RN license in the State of New York.
