His troponin level is 30, The last coronary angiogram (CAG) done in April 2024 showed mild coronary artery disease (CAD). Echocardiogram: Normal LVEF, apical wall HK , no significant Valvular heart disease First ECG in ER What is your diagnosis? In Type 1 Diabetes, Pancreas Size May Predict Disease Progression The size of the pancreas may be a marker for the risk of disease progression in patients with type 1 diabetes, finds a new study published in Diabetes Care, the journal of the American Diabetes Association. Researchers at the Vanderbilt Diabetes Research and Training Center (DRTC) have found that a smaller pancreas leads to faster progression to stage 3 type 1 diabetes. Extracranial internal carotid artery stenosis (CAS) is a major cause of ischemic stroke, and it is estimated to cause 8–15% of such events. A strong association between the presence of carotid atherosclerosis (CA) and cardiovascular events has been shown in individuals with diabetes. In a recent article by U.S. News, experts highlighted the strong link between gum disease and diabetes. Diabetes increases the risk of periodontal disease by weakening the immune system and raising inflammation, while severe gum disease can disrupt insulin sensitivity, making diabetes harder to control. This creates a dangerous cycle that affects both conditions. Treating gum disease can help manage diabetes. In patients with Chronic Kidney Disease (CKD)-5 and type 2 diabetes mellitus, SGLT-2 inhibitors are associated with a lower risk of 👉Dialysis initiation 👉Hospitalization for heart failure 👉Acute myocardial infarction and 👉Acute kidney injury without difference in all cause mortality. This is a Taiwanese retrospective study on a large database but there have been no RCTs to support these findings. The observed findings provide hope to delay dialysis initiation on our patients approaching end stage renal disease. Keep that ‘gliflozin’ going. #ACPIMPhysicians #ACP #Internalmedicine
