I worked for several organizations that drove for perfect understanding of technical "root cause," - spending up to millions of dollars - but totally ignoring the organizational, social, funding, and human factors that allowed the problem to persist to the point of causing the failure, or missed opportunities to avoid the failure. The one FRB I led, I included a new category of these contributors, but the customer was not impressed. Their (non)management of the system had led to the failure, they did not want to hear this answer. And having an outside advisor run the FRB was just a fig-leaf (for who I never figured out) - they just wanted a report to file. When an organization has a new leader, and a new failure, who wants the new boss to hear that the REAL root cause was the organization's prior behavior? Much easier to blame it on a technical issue, or a single person's actions. And unwritten rule of organizations is that they will protect themselves. But, there is ALWAYS another layer to the onion, so who is to say stop here, this really is root cause? My prior paper on a specific instance was not well received, for reasons above.
