The recent Lord Darzi report from Department of Health and Social Care on the state of the #NHS since 2001 highlights critical challenges facing the UK’s healthcare system. The findings are sobering: From the sharp rise in long-term conditions to stagnation in clinical care outcomes and increasing operational inefficiencies, it is clear that transformative action is needed to address these issues and improve the health of the nation. At CFAR, we know that governance structures in healthcare systems are not one-size-fits-all. Different boards will face different challenges depending on their unique context. We worked with a post-merger health system whose primary aspiration was to adjust the decision rights between the system board and the subsidiary boards. This starkly contrasts another board rethinking its practices due to a significant shift in its strategy and the need for stronger academic and community partnerships. To ensure a governance refresh is effective in either case (or the many cases in between), it will be important to understand the current state of the organization and its governance, identify its aspirations for the future, and design structures that will support the intended change. We find that opportunities to strengthen board effectiveness are generally found in four areas: 1. Membership and composition 2. Individual members' behavior 3. Scope of deliberation 4. Some thoughts on how we might approach solving the immense challenges highlighted in Lord Darzi’s report… We often talk about the importance of ‘data’ in solving the complex crises that face our nation’s health, today. But too often, we place the emphasis on access alone. Access is only one piece of the puzzle when it comes to data-driven solutions. Another critical element is being able to discern the relevant data points in a sea of information and to be able to structure, link and translate those data points into usable datasets and digestible narratives that can be used to solve problems. Investment into data utility and enablement is something I don’t think we discuss enough in the context of harnessing the UK’s valuable health data to support us in addressing the painful deficits that Lord Darzi’s report describes. Developing a culture of data-driven research in clinical settings and supporting early stage researchers to develop data science skills is critical to helping translate the wealth of health data that exists within the UK. And so is developing deeper, more sustainable industry partnerships, where consistent investment into mutually beneficial research and development areas can support the collection, curation, and even generation of meaningful, context-sensitive data which can support well-evidenced and validated innovation. There is extraordinary opportunity for partnerships where industry expertise is combined with deep clinical understanding and health system knowledge, underpinned by commercial sustainability, and informed by the right data to solve the problem. But reducing research relationships to data transactions is a risk we run when we consider data access above data utility. Data doesn’t solve problems alone - but combined with knowledge, context and expertise, and translated into evidence, discoveries and findings, it can.

Anyone who follows the topic of health system transformation and #vbhc will know the importance of clinical engagement. It doesn't matter whether we are talking about remodelling whole care pathways or reducing unwarranted variation in care, or stewardship. Strong clinical leadership, ownership and involvement with finance and managerial colleagues is critical for success. Allan has thought more deeply about this than possibly anybody else I know. This will be a masterclass in how clinical networks can drive #vbhc and learning health and care systems at national and regional level.
