Ahead of print Impact of intravenous infusion of lidocaine on intrapulmonary shunt and postoperative cognitive function in patients undergoing one-lung ventilation Intravenous infusion of lidocaine as an anesthesia adjuvant can improve patient outcomes, but its impact on intrapulmonary shunt during one-lung ventilation (OLV) has not been clarified. The objective of this study was to determine the effect of intravenous lidocaine infusion on intrapulmonary shunt during OLV and postoperative cognitive function in video-assisted thoracoscopic surgery (VATS). https://lnkd.in/dbpH6G_G

Tuesday concluded Manitoba’s first vNOTES (Vaginal Natural Orifice Transluminal Endoscopic Surgery) case. Dr. Anushuya Kasi and Dr. Rutherford successfully performed Manitoba’s first vNOTES case at Brandon Regional Hospital. Thanks to both of them for starting Manitoba’s journey with such a successful vNOTES Hysterectomy. vNOTES is an innovative, minimally invasive approach enabling access into the peritoneal cavity through the vagina, effectively combining the benefits of laparoscopic and traditional vaginal surgery. It is conducted fully through the natural orifice of the vagina with the laparoscope being utilized for visualization. vNOTES studies have pointed to less post-op pain, reduced OR time, faster recovery times and less procedural anesthesia. Driving Pressure During General Anesthesia for Minimally Invasive Abdominal Surgery (GENERATOR) — study protocol of a randomized clinical trial GENERATOR (https://lnkd.in/eWnMqNqJ; https://lnkd.in/dzQX3Api) will be the first adequately powered randomized clinical trial to compare the effects of individualized high PEEP with RMs versus standard low PEEP without RMs on the occurrence of PPCs after minimally invasive abdominal surgery. The results of the GENERATOR trial will support anesthesiologists in their decisions regarding PEEP settings during minimally invasive abdominal surgery: https://lnkd.in/dPyTwW-P Sunny Nijbroek, Liselotte Hol, Galina Dorland, Tom Vermeulen for the PROtective Ventilation Network (https://lnkd.in/d78adsXF). #AmsterdamUMC #ZonMW #PRotectiveVEntilationNetwork #PROVENetwork #Anesthesia #IntraoperativeVentilation #Ventilation #PEEP #RecruitmentManeuvers #PostoperativePulmonaryComplications #PPC #RandomizedClinicalTrial

Among patients undergoing aortic arch surgery, a higher CPB hematocrit level is associated with reduced likelihood of postoperative ARF. Preoperative renal function, but not hypothermic circulatory arrest duration, significantly modified this association. The maintenance of higher CPB hematocrit may reduce the incidence of postoperative ARF, especially for patients with poor preoperative renal function.

Prolonged times to tracheal extubation are caused principally by high (>0.5) age-adjusted minimum alveolar concentration (MAC) of the inhalational agent at the end of surgery. We performed a prospective observational study. A major cause of high MAC achieved at the end of surgery was a high MAC goal chosen by the anesthesia practitioner when surgical closure started. The goals were often higher or lower than could be based on earlier pharmacological studies. https://lnkd.in/g2XZ3J5m

Preoperative estimated glomerular filtration rate to predict cardiac events in major noncardiac surgery: secondary analysis of two large international studies. Our paper on eGFR in predicting events post noncardiac surgery has just been accepted in the British Journal of Anesthesia. Do look out for the title and interesting revelation here and on the journal site. Perioperative cardiac events are preventable, if we can monitor our patients for certain signals. These events can be fatal if we do not pick them up and manage them appropriately. 💐
