This #EndometriosisAwarenessMonth, be prepared to talk to your health provider about #endometriosis. You can ask questions like: > How can I know if what I am experiencing is endometriosis? > What are the different treatment options available for someone with endometriosis at my age? > What are things I need to consider about my endometriosis as I get older? The Endometriosis Teen Toolkit by SWHR has suggested questions and patient worksheets to help track your health care appointments. According to the National Council on Aging (NCOA), nearly 95% of adults 60 or older have at least one chronic health condition, with nearly 80% having two or more. With an increase in multiple medications, without a clear understanding of how they interact, patients can increase their risks for a hospital visit for delirium. At CHE we are a premier provider of behavioral health services and the partner of choice for over 1,000 post-acute care facilities. We provide a unique programmatic approach to behavioral health services focused on best clinical care practices, interdisciplinary support, and assisting facilities in meeting regulatory compliance. https://lnkd.in/ehUjaitZ #ChronicHealth #SeniorHealth #BehavioralHealth #PatientCare #HealthAwareness #MedicationManagement #HealthcareProviders #AgingWell #PostAcuteCare

QUESTION? Are we tracking the impact of physical activity prescriptions from GPs effectively? As we continue exploring ways to boost physical activity engagement through community and healthcare initiatives, I’m left wondering: Do we have reliable data on how many GPs in Scotland are actually prescribing physical activity interventions and what impact this is having? Physical activity is a powerful tool for improving both physical and mental health, yet we rarely discuss the extent to which our primary care practitioners are leveraging it as a "prescription." If we truly want to make physical activity a cornerstone of preventive health, it’s crucial to understand: How many GPs are actively prescribing physical activity as part of their patient care? What kind of impact are these interventions having on patient outcomes? Are there regional differences or best practices we can learn from? I’d love to hear from those in the healthcare and public health sectors: Is this data readily available? If not, how can we better track and support the role of GPs in promoting physical activity as a key intervention? #PhysicalActivity #GPReferrals #PreventiveHealth #PublicHealthScotland #DataInsights

Recent insights from the Canadian Institute for Health Information highlight critical trends in emergency department visits. A significant portion of patients—particularly those with mental health and substance use issues—frequently visit EDs, pointing to unmet care needs in the community. These statistics emphasize the urgent need for continuous, accessible care to address complex health and social needs. While improving primary care access is important, a more comprehensive, collaborative approach is needed to effectively tackle ED overcrowding. As we explore community-based solutions, collaborative care models and virtual healthcare could play a significant role in reducing unnecessary ED visits and improving overall health outcomes. How do you think more coordinated outpatient care could improve this critical situation? #EmergencyCare #CollaborativeCare

The text discusses the inadequate incorporation of family and caregiver spillovers in health economic evaluations. It highlights the minimal consideration of caregiver impacts in cost-utility analyses and presents findings from various studies reflecting this omission. The Spillovers in Health Economic Evaluation and Research (SHEER) task force aims to address this by defining family and caregiver health spillovers and providing specific recommendations. Implementing shared-savings models in behavioral health could help bridge the gap to full value-based care, making the shift more manageable for providers. These models reward cost savings tied to patient outcomes to ease the financial risk while improving care quality. For providers and payers alike, this approach offers a balance of risk and reward. #ValueBasedCare #VBC #Healthcare https://lnkd.in/e8gh2VwF
