In collaboration with Reto Kaderli, MD, FEBS and Rowan Iskandar, we recently developed a new decision-analytical model for parathyroid surgery that is now published in BMJ Open. Such early health technology assessments are useful evidence-based methods to address go/no-go decisions and rationalize R&D resource allocation. As available resources are scarce during the early phases of development, it is even more critical to weigh the expected benefits of an innovation to the developmental and opportunity costs. Interestingly, such tools are rarely applied in early translational research. In the context of parathyroid surgery, our study highlights the importance of weighing the a priori prevalence of disease types and finds that preoperative localization modalities should maximize specificity (minimize false positives) to avoid unnecessary invasive surgery. We have published a paper on the role of robotic Surgery in Cystectomy for endometrioma. "Ovarian Loss in Laparoscopic and Robotic Cystectomy Compared Using Artificial Intelligence Pathology" There is evidence that there is ovarian and follicular loss during during cystectomy for endometriosis. In our Pilot study, we found that there is significantly less loss of ovarian loss during robotic Cystectomy especially in cases of large cysts or bilateral cysts when compared to cystectomy done with laparoscopy. To our knowledge, this comparison has not been done before. This is a landmark study. There are still gaps and needs in the Minimally Invasive tool Armamemtarium. It’s really not perfectly equipped yet. Thankfully there are companies who listen, and cooperate in the development of meaningful everyday solutions, to untie our hands, give us better tools for easier and more efficient heart surgery, such as GEISTER. Kudos to Geister’s responsiveness , we now launched a modified Cobra clamp, for both open and minimally invasive heart surgery. The pain-point? You know how the traditional cobra hangs in the way, and obstructs your field, particularly in Minimally Invasive CABG, where it dangles in your way and obstructs much of your work port, when you’re trying to do proximal anadtomoses? That’s because it features a very acute angle and because it’s very short. Hence , we developed a longer and flatter Cobra proximal anastomosis clamp - the so called “Kofidis type”- that removes those limitations. Both at NUS (where this idea was conceived), as well as the International Heart Surgery Center of Greece at Hygeia Hospital Athens (where new applications and methodology is being established), the wheels of Innovation are turning.
