Transcatheter aortic valve replacement (#TAVR) with a supra-annular, self-expanding valve (SEV) was noninferior to TAVR with an intra-annular, balloon-expandable valve (BEV) with regard to clinical outcomes, and superior concerning bioprosthetic valve dysfunction, through 12 months in patients with symptomatic, severe aortic stenosis who have small aortic annuli, the SMART Trial shows. #ACC24

Radioligand therapy has added to an expanding list of therapeutic options that have already improved survival in mCRPC, Sathekge and co-authors noted in their introduction to the study. Angiographic-derived functional assessment (eg. QFR) has demonstrated a high accuracy in determining the functional relevance of coronary stenoses. It is also useful in planning PCI, utilizing the virtual stenting tool. Here’s a recent case of a middle-aged male with dyspnoea. Resting echocardiogram showed moderate LV dysfunction (EF40%), with LAD territory RWMA. After commencing medical therapy, a coronary angiogram showed moderate-severe LAD disease. QFR which was done within 1 minute with no invasive wire or adenosine confirmed functional significance, with 2 lesions requiring attention. Successfully treated with a hybrid DES/DCB approach using IVUS guidance. Post PCI QFR very reassuring at 0.94.

Endovascular thrombectomy for acute ischaemic stroke with established large infarct (TENSION): 12-month outcomes of a multicentre, open-label, randomised trial. Interpretation In patients with acute ischaemic stroke from large vessel occlusion with established large infarct, compared with medical treatment only, endovascular thrombectomy was associated at 12 months after stroke with better functional outcome, quality of life, and overall survival. These findings suggest that the benefits of endovascular thrombectomy in patients with an ischaemic stroke and a large infarct are sustained in the long term and support the use of endovascular thrombectomy in these patients. #TENSION #Stroke #endovascular #treatment #LVO

Radial EBUS/GS via a therapeutic scope is still our first choice whenever feasible for larger and middle third target. The large GS allow passage of 1.7mm forceps and able to tamponade bleeding effectively, allow us to complete nav to biopsy within 10 minutes usually. In this case 7.55 minutes.

Good news for patients with tricuspid regurgitation: the latest insights from the TRI-fr trial presented at the #ESCCongress show once again that treatment with the #TriClip Transcatheter Tricuspid Valve Repair System is superior to medical therapy alone. Key takeaways: - In severe tricuspid regurgitation, the tricuspid valve does not close properly and this can alter kidney and liver function, and cause disabling symptoms. -T-TEER plus optimal medical therapy resulted in a significant improvement of the primary endpoint including death, cardiovascular hospitalisation, functional class and patient-reported outcomes. - T-TEER plus optimal medical therapy was associated with major quality-of-life improvements.
